COVID-19 Second Booster Dose
The CDC recently released a recommendation for a second COVID-19 booster dose for certain persons. Specifically, a second mRNA booster dose could benefit people ages 50 years and older, as they are at increased risk for severe COVID-19. People ages 50 years and older may choose to receive a second booster dose, if it has been at least 4 months after the first booster. We recommend that appropriate residents receive their second booster dose. The COVID-19 vaccines (Pfizer or Moderna) are available. Please let your physician or nurse know right away if you or your loved one would like to receive the second booster.

Susan L. LaNinfa CNHA, CALA, FACHCA
AdministratorAn Administrator since 1990, Susan has lived in Rhode Island for over 32 years. A graduate of Providence College and Bryant University, Sue has a BS in Health Administration, her MBA and a CAGS. A member of the American College of Health Care Administrators and licensed as a Nursing Home Administrator in all New England states and Florida, Susan has worked in skilled nursing centers and assisted living in Rhode Island, and Massachusetts, Connecticut and Vermont. Susan’s personal belief; we are entrusted to care for people who deserve kind, compassionate people to respect, honor and help them achieve the pursuit of joy, love and happiness.

Fabian Delgado, MD, FACP, CMD
Medical DirectorDr. Delgado fluent in both English and Spanish, graduated with his medical degree from the Catholic University School of Medicine in Guayaquil, Ecuador. A Board Certified Internist, he completed his residency in Internal Medicine at the University of Kansas Medical Center and successfully completed a Fellowship for Healthcare Leaders at the University of California, San Francisco. He is a Fellow of the American College of Physicians, and has extensive experience in medically complex patients in all settings: Acute care, Long-Term Acute Care, Post-acute care and Hospice care. Additionally, Dr. Delgado is a Certified Medical Director by The American Board of Post-Acute and Long-Term Care Medicine and is passionate about quality improvement in post-acute care.
Dr. Delgado has served in other leaderships roles as the Medical Director of multiple post-acute care facilities in Massachusetts and Rhode Island and has supervised medical residents at TUFTS Medical Center, Transitional Year Residency Program at Brockton Hospital. Currently affiliated with Vantage Healthcare, an experienced, high-performing physician practice for seniors.
As the Medical Director of Warren Skilled Nursing & Rehab Center since 2020, Dr. Delgado compassionately directs and delivers high-quality healthcare within our family oriented team setting. And whenever time allows, he enjoys travelling, dining, soccer, American football, baseball and mixed martial arts.

Marguerite Morris, RN
Director of NursingMarguerite graduated from St. Joseph School of Nursing with a RN diploma in 1988. She started her Nursing career working at Kent County Hospital on the Med Surg/Oncology Unit. She switched to Long Term Care Nursing in 2004 working as a charge nurse for Genesis HealthCare. She became a Unit Manager in 2006, was promoted to Assistant Director of Nursing in 2012 and finally to Director of Nursing in 2014. She enjoys being a part of an amazing family orientated team, working together for the well-being of our residents and their families.

Sherrill Ferreira, LPN
Admissions DirectorSherrill Ferreira has been the Director of Marketing and Admissions at the Warren Center since 2004. Her experience as a Licensed Practical Nurse allows her truly understand the unique needs of each prospective resident. She is compassionate and kind and is well-known in the local community as an active member of the Chamber of Commerce and the local Rotary Club.

Hannah Allain, COTA/L
Rehabilitation Program DirectorHanna joined the Warren Center team in 2018. She earned her degree from Bristol Community College and completed her internship at Butler Hospital, Geriatric Unit. Hanna is currently enrolled in the University of Massachusetts at Dartmouth's Health Care Management Program.

Sue Makuch
Social WorkerSue has been a social worker since 1993 with the majority of her career spent in hospice. Three years ago she made a career change to working in long term care and is currently the director of social services at Warren Center. She is a loving mother of two young women, Samantha and Emily. She also has two kitties, Delilah and Leila. In her spare time, when the weather is nice, you will find her on her motorcycle (if you’re fast enough). She is currently studying sound healing and is certified and actively practicing reflexology and reiki. If Sue could, she would heal the world.
Privacy and safety
A great job is being done to accommodate both our father and us as the visitors. Privacy is given to us during the visit while the staff is still able to assure all safety measures are kept. The visiting area outdoors was as cool as it could be during the heat wave and we enjoyed our visit. Great job!!!

Excellent rehab and good meals
My experience at the Warren Skilled Nursing & Rehab Center has been a very positive one. Meals have been overall very good. Nursing and rehab has been excellent. Staff members were very understanding, caring and compassionate.

Wonderful staff, PT and activities
Hurting my ankle has not been bad for me. The staff at Warren Skilled Nursing and Rehab Center have been wonderful. I enjoy the activities. Physical Therapy has been very hard but worth the work out. Occupational therapy is excellent too. The food is good, hot and nice tasting. The nurses are on their toes to answer the light for all of my needs.

I’m trying to find the words to thank you all for your patience and kindness with Pauline during these past few months. I hope I have the opportunity to see you all in person one more time to express my deep appreciation. Everyone…The Nurses Mary, Angie and Sue, Sue LaNinfa, Marguerite, Lexi at the door who always greeted me with a smile. Sonni, who walked me through the absolute nightmare of applying for Medicaid. The sweetest girl ever who arranged all the extra curricular activities, and the maintenance man, for his patience.. You are all over worked, under paid but so very much appreciated by me and my family. I’ll try to drop in one of these days just to say a quick hello…from my heart, thank you All.
