COVID Booster Shots
Hard to believe we are already half way through the summer! In preparation for the fall COVID and Flu season and with the new COVID BA.5 variant circulating, we are recommending all nursing home and assisted living residents receive their COVID booster and Flu shots. Centers will provide access to Flu shots in September and boosters are available now.

Michael Herald
AdministratorMichael is a a graduate of Marshall University and West Virginia University with Master's degrees in Health Care Administration and Corporate Communications. He has over 20 years of professional experience in both Long-Term and Acute Care Operations. He is active in the West Virginia Healthcare Association, his local community and has a passion for working with the geriatric population.

Dr. Jeffrey Holmes
Medical DirectorDr. Jeffrey Holmes is our Medical Director and joined the Center in 2016.

Kristen Shamblin
Nurse PractitionerKristen Shamblin is our Nurse Practitioner and joined Putnam Center in 2011. She graduated from Marshall University in 2003 and 2008 with a Bachelor of Science in Nursing and Master of Science in Nursing with Family Nurse Practitioner emphasis. She is a Board Certified Nurse Practitioner with over 10 years of long term care experience. She also has additional education in dementia, geriatrics, palliative care and wound care.

Amanda Blackshire
Director of NursingAmanda received her Associates Degree in nursing in 2019 from Bridge Valley Community and Technical Center. Currently she attends WVU - Parkersburg to obtain her Bachelors of Science Degree. She started her career for the company in 2003 as a CNA. From there she became an LPN and currently RN. She has 19 years of experience in long term care. Amanda works hard for her residents and fellow co-workers. She is a Leader and Team Player and has everyone's interest at heart.

Freda L. Bird
Senior Admissions DirectorFreda has worked in skilled nursing and long-term care since 2009 with the Genesis HealthCare Facilities. She started at Valley Center in So. Charleston as the Admissions Assistant in April of 2009 and then was promoted to Admissions Director for Heritage Center in Huntington on Oct. 2011. She then accepted the DMA Role at Putnam Center in Jan. 2014 and then was promoted to Sr. Director of Marketing and Admissions on Feb. 2017. She brings 30 years of marketing to the local community. She is committed to keeping her residents and rehab patients satisfied and informed.

Sara Parsons
Rehab/Specialty Program DirectorSara received her Master’s Degree in Physical Therapy from West Virginia University. She has over 10 years of experience in skilled nursing. Sara became the Program Manager at Putnam in March 2006. She takes pride in being a patient advocate and helping all patients reach and maintain their highest functional level.
