Pueblo, CO - April 27, 2021 - Telligen QI Connect™ announced today that Pueblo Center, a Genesis HealthCare affiliated skilled nursing facility located in Pueblo, CO, was named a Telligen QI Connect Community Quality Champion for its work to join together with individuals and partner organizations to focus on the most challenging issues related to COVID-19 and beyond. Areas of focus include opioid misuse, increasing access to behavioral health services, ensuring smooth patient transitions with changes in level of care, patient safety, chronic disease prevention and self-care, and nursing home quality.
