COVID Booster Shots
Hard to believe we are already half way through the summer! In preparation for the fall COVID and Flu season and with the new COVID BA.5 variant circulating, we are recommending all nursing home and assisted living residents receive their COVID booster and Flu shots. Centers will provide access to Flu shots in September and boosters are available now.

Amanda Nelson
AdministratorAmanda Nelson first joined the company team in 2019 at Sandy River in Farmington, and then transferred to Oak Grove in 2021. With a long career in healthcare and her background as an occupational therapist, Amanda demonstrates a strong commitment to interdisciplinary collaboration to provide the best resident-centered care. Amanda is passionate about supporting staff in their day-to-day needs, as well as helping them to grow into the leaders of the future. Amanda is an active member of the American College of Health Care Administrators and was awarded the Eli Pick Leadership Award for high performance in quality measures, census management, and annual survey in 2018. She is also active with the American Health Care Association and earned the Bronze Award for Excellence in 2020.

Dominique Velazquez
Admissions DirectorDominique all together has worked for Oak Grove for 4 years. Her journey started as a CNA, Medical Assistant and now Admissions Director. Dominique has an associate’s degree in science and soon will be graduating with her bachelors in science with a minor in business. Dominique was born in Waterville and has always had the passion for helping her community. You will always find Dominique dedicated to making residents stay and experience above and beyond.

Sarah Johnston
Director of RehabilitationSarah has worked as an Occupational Therapist for 10 years and has been with us for 3.5 years where she was the Director for skilled rehabilitation and long term care services. Sarah received her Bachelor's Degree in Social & Behavioral Science as well as her Master's Degree in Occupational Therapy from the University of Southern Maine. Sarah was born/raised in Richmond Maine and aspired to work with children as a pediatric therapist, which she did, but found a new passion for geriatrics while filling in for a colleague at a small local nursing home. Sarah is committed to providing the most comprehensive care to her patient's while supporting their rehabilitation goals.
My Mom did several rehab stints at Oak Grove over the years and spent the last year of her life there as a resident. The staff took wonderful care of her and were patient with her throughout each of her stays! They went out of their way during the Covid lockdown to provide ways for us to be in touch with her and provided the nurturing we couldn't. She always raved about how great the food was!
