Milford Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care. We also offer a dedicated memory support unit for residents with Alzheimer's or dementia. We accept Medicare, Medicaid, and most private insurances. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. Milford Center is a We Honor Veterans community partner!

Dr. Alvaro Toledo Rivera
Medical DirectorDr. Alvaro Toledo Rivera, MD is a Hospitalist at Bayhealth Kent Campus (formerly Kent General) and was named Medical Director at Milford Center in April of 2021. Dr. Toledo graduated from medical school at Universidad Industrial de Santander in Colombia and did his residency in Internal Medicine at Geisinger Medical Center in Danville, PA. He is also currently certified with the American Board of Internal Medicine. In 2019, Dr. Toledo was named Physician of the Year at Bayhealth Kent Campus and has recently served as a member of their COVID-19 task force. In light of the potential harm the pandemic poses for nursing home residents in particular, Dr. Toledo Rivera is vigilant in ensuring the safety and health of Genesis Healthcare’s patients. Paige Cathell, BSN, RN, WTA, CDE, NPE, IP, CNE
Director of NursingPaige is a Delaware local both born and raised in Sussex County. In 2012, Paige received her undergraduate degree in Psychology from Salisbury University. In 2014, she received her Nursing degree from Delaware Technical & Community College. She went on to pursue a Bachelors of Science in Nursing degree from Wilmington University in 2016. Most recently in 2020, she continued her education to include a Masters of Science in Nursing degree also from Wilmington University. Paige holds certifications in Wound Care from the Wound, Ostomy, and Continence Nurse Society (WOCN), Diabetes Care and Education Specialist from the National Certification for Diabetes Educators (NCBDE) and Infection Prevention from the CDC. In her free time, Paige enjoys spending time with her family and traveling to new destinations. Paige resides in Laurel, DE with her husband Chad and son Mason.

Erica Mansfield
Director of Admissions & MarketingErica started at Milford Center in November 2018 as a part time receptionist. During that time she was asked to assist in the Admissions Department as the Assistant. She quickly learned the admissions process and was promoted to full time. Erica is a Delaware native and lives in Harrington with her family. Placing your loved one in a nursing facility can be overwhelming, and she strives to make that transition as smooth as possible for both the patient and family.

Susan Faulkner
Director of RehabilitationSusan Jordan, the Director of Rehabilitation, has been with Milford Center since September 2019 and with Genesis Rehabilitation Services since 2009. Her career in rehabilitation began in 2006. Susan was born and raised on the Eastern Shore of Maryland and attended Salisbury University and Anne Arundel Community College where she received her Physical Therapist Assistant Degree. She enjoys the outdoors, going to the beach, sports, spending time with her dog, Raven and cat, Camo. Susan and her team are committed to ensuring quality treatments for successful recovery for transition to home or the next level of care.

Amy Ferguson-Moore
Dementia DirectorAmy Ferguson-Moore is a Certified Dementia Practitioner and Alzheimer's Association trained Support Group Facilitator. She has approximately 20 years experience working with individuals diagnosed with dementia. She joined the team at Milford Center in 2019 as the Dementia Program Director. She enjoys volunteering on the committee for the Rehoboth Beach Walk to End Alzheimer's as the Logistics and Social Media chair. She is committed to ensuring the highest quality of care for residents of the Memory Support unit.
