Memory Support of Heritage Hall is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care services. We also have a specialized memory support unit for those residents with Alzheimer's or dementia. We accept Medicare, Medicaid, and most private insurances. Memory Support of Heritage Hall is an AHCA Bronze Quality Award winner, a WE HONOR VETERANS partner and is part of the Heritage Hall Health & Rehabilitation Campus, which also offers transitional care and on-site dialysis in adjacent buildings. Physician Services on the campus provided by Post-Acute Care Clinicians.

Dr. Rami Ashkar
Medical DirectorDr. Ashkar was born and raised in Syria. He graduated from Damascus University Faculty of Medicine in 1995 and conducted his residency in internal medicine at Lutheran Medical Center in Brooklyn, NY. Upon the completion of his residency, he continued his work in the emergency room while conducting research at Mount Sinai Medical Center. Dr. Ashkar relocated to Western Massachusetts and joined Hampden County Physicians Associates as well as became a hospitalist at Mercy Medical Center. In 2019, Dr. Ashkar joined Post Acute Care Clinicians, LLC., while continuing to work at Mercy Medical Center on per diem basis. In his spare time, Dr. Ashkar enjoys spending time with his family, reading and sports. He presently serves as the Medical Director for Heritage Hall West, South and Memory Support of Heritage Hall. Dr. Ashkar works closely with our interdisciplinary team to deliver the finest care possible to our residents. He is committed to providing the best outcome possible for his patients. Stephanie Schellenger MHS, CDP
Dementia Program DirectorStephanie started her career in long term care on the Heritage Hall campus in 2015. She worked in recreation and her passion for helping those diagnosed with dementia was born. She received a Bachelor’s Degree in Social Sciences from American International College and earned her Master’s in Human Services from Capella University with a focus in Gerontology. Stephanie became a Certified Dementia Practitioner in 2018 and provides Dementia training to all Heritage Hall staff. She is also a committee member of the Alzheimer’s Association of the Pioneer Valley.
Kimberly Moran
Director of RehabilitationKim graduated from Springfield College with a Certificate of Graduate Studies (CAGS) and Masters of Science in Occupational Therapy. Kim has been employed with Genesis Rehab Services for 22 years. When Kim is not caring for patients she enjoys home renovation projects, boating and spending time with her family and friends.

Bolaji Mustapha LNA, MBA
AdministratorBolaji Mustapha is a seasoned Administrator currently working at Memory Support of Heritage Hall in Agawam Massachusetts as the Center Executive Director. He completed his Bachelor degree in Accounting in Nigeria, and MBA at the American International College, Springfield MA. He have been working in Long-term Care in various capacity for almost 20 years of which a decade of it was as an administrator with a tracking records of excellence. Bolaji Mustapha start his career as Certified Nursing Assistant in 2005 where he developed great passion for the care of the aging population. He is very compassionate, kind, and enjoy the caring for ageing population, and his humble background made his and colleagues love his team orientation. Bolaji Mustapha is not just an administrator but currently a license nurse, and with a background in Accounting make him very successful Administrator. In the year 2020, he was presented with an award for the Leader of the Year. His leadership skill has been an aspiration to friends, family, residents, and colleagues.
Wonderful care
My Aunt was at Heritage Hall East (currently Memory Support of Heritage Hall) for short-term rehab. She was there for a little over 2 weeks. She has dementia and can be very forgetful. The center did a wonderful job taking care of her.

Wonderful and very friendly
My mother was at Heritage Hall East since end of November . She had dementia and it was difficult for us during this time with the virus. I had cared for her the past three years at home so it was big change for Mom. But THANKS to all the staff on B wing they took great care of Mom. We had weekly zoom calls with mom and the family and those calls with mom laughing with us are the memories we can hold on to now. Mom would also say to us many times "the staff are wonderful and very friendly ".

Great place, staff always give a warm welcome when you go visit family. Recreation staff does a great job keeping everyone happy and entertained! Great nurses and top notch care for those suffering from dementia, all around excellent facility!

Love and Care
The center is clean, organized and residents are well cared for! The staff all show respect and are diligent in maintaining the safety protocols; especially with COVID! We’re glad Mom was sent there and are pleased with the Love and Care given to her!

Care and compassion
I am so impressed with the care and compassion my mother has received since being at Memory Support of Heritage Hall! She came for rehab and I was so impressed with the care that the decision was made to make it her permanent home. They have made her transition extremely easy considering how difficult it is for people with dementia to adjust to unknown surroundings. The staff is very helpful and always available to answer any questions and offer support in any way they can.

The staff is professional, caring and very responsive
My mom has lived with me for the past nine years. Over the nine year period she started to develop dementia. It eventually became a fact that she needed more around the clock care at this time than we could give her at home. It was hard for me to separate from my mom and send her to a nursing home facility. I inquired with the Genesis HC Memory Support Center in my home town of Agawam. After meetings with Genesis HC I reluctantly placed my mom in their Long Term Care Memory Support Center because I didn't think I could trust anyone with her. I was wrong because the staff at Genesis HC Memory Support Unit in the East Building has left myself and our family with a great feeling of comfort and trust that I didn't think would be possible. The staff is professional, caring and very responsive to the needs of my mom and she is receiving great professional care and the TLC that she so dearly needs something that I didn't think she could get from anyone but our family. Kudos to this great team of individuals who all play a part in providing my mom with the care and stability she needs.
