Heritage Woods
462 Main Street, Agawam, MA 01001
Heritage Woods is an Assisted/Senior Living Community offering residents a dynamic lifestyle that promotes mind, body and spirit wellness. Proudly serving our community for 24 years! Registered as an Eden Alternative® community for longer than any other Assisted Living Community in the country, Heritage Woods is focused on alleviating the three plagues of the elderly: loneliness, helplessness, and boredom. It is our mission to empower our residents by encouraging participation in various committees and groups, and by planning our activities. The Eden Model motivates residents to maintain their independence and be the decision makers who engage in meaningful activities of their choosing. As part of our holistic approach to maximizing quality of life, pets, plants, and children are at the forefront of Heritage Woods, and are monumental in combating the three plagues.
At Heritage Woods, we are able to offer a variety of sizes and styles of apartments to fit the unique social and financial needs of the prospective resident. Companion suites are also available. Senior Living is the preferred lifestyle for seniors who value their independence but may benefit from additional support to maintain it. Our residents enjoy many amenities including restaurant style dining, a wide array of social, educational and spiritual activities and entertainment, transportation to medical appointments, housekeeping and laundry. In addition, on site therapy and physician services help residents to have all their needs taken care of in one place.
Heritage Woods is the first stand-alone Assisted Living Community in the nation to earn the Step II Quality Award from the American Healthcare Association/National Center for Assisted Living.
In addition to our assisted living apartments, Heritage Woods offers a cozy, secure memory support community with 18 private apartments and a beautiful enclosed courtyard with gorgeous flowers and raised garden beds. Our memory support community offers residents specialized programming and meaningful activities, all overseen by our dedicated Memory Support Director.
Dementia Care Education by Genesis HealthCare is recognized by the Alzheimer’s Association® for incorporating the evidenced-based Dementia Care Practice Recommendations in the following topic areas: Alzheimer’s and dementia, person-centered care, assessment and care planning, activities of daily living, and behaviors and communication.
Heritage Woods is part of the Heritage Hall Health & Rehabilitation Campus, which offers a continuum of skilled nursing and rehabilitation services. The campus specializes in post-hospital rehabilitation, memory support and on-site dialysis in adjacent buildings. Our Senior Living and Rehabilitative Health campus supports our residents to maximize their quality of life while enjoying the peace of mind knowing additional services are available onsite should needs arise.
Our 3 day minimum Respite stay offers the flexibility for Caregivers to take a much-needed break from the demands of daily life, while providing an opportunity for your loved one to socialize with an array of activities in a safe, supportive, welcoming environment.
