Heritage Hall South is a skilled nursing facility offering post-hospital and short-term rehabilitation, as well as long-term and respite care services. Heritage Hall South also offers on-site dialysis services through the dialysis center attached directly to our building. We accept Medicare, Medicaid, and most private insurances. Heritage Hall South is an AHCA Bronze Quality Award winner and is part of the Heritage Hall Health & Rehabilitation Campus, which offers memory care and transitional care in adjacent buildings. Physician services on the campus are provided by Post-Acute Care Clinicians.

Kevin Kaczynski MS, LNHA
AdministratorKevin is a graduate of Salve Regina University with a Master’s Degree in Healthcare Administration and Management. He attended the University of New Haven and has been licensed as a Nursing Home Administrator since 2012. Prior to transitioning to long-term care, Kevin worked in home healthcare administration.

Dr. Rami Ashkar
Medical DirectorDr. Ashkar was born and raised in Syria. He graduated from Damascus University Faculty of Medicine in 1995 and conducted his residency in internal medicine at Lutheran Medical Center in Brooklyn, NY. Upon the completion of his residency, he continued his work in the emergency room while conducting research at Mount Sinai Medical Center. Dr. Ashkar relocated to Western Massachusetts and joined Hampden County Physicians Associates as well as became a hospitalist at Mercy Medical Center. In 2019, Dr. Ashkar joined Post Acute Care Clinicians, LLC., while continuing to work at Mercy Medical Center on per diem basis. In his spare time, Dr. Ashkar enjoys spending time with his family, reading and sports. He presently serves as the Medical Director for Heritage Hall West, South and Memory Support of Heritage Hall. Dr. Ashkar works closely with our interdisciplinary team to deliver the finest care possible to our residents. He is committed to providing the best outcome possible for his patients. It is an honor to have Dr. Ashkar on our team. Tina Eggleston, RN
Director of NursingTina spent nearly two decades as an LPN Unit Manager for a subacute unit prior to becoming an RN. She graduated from Southern Vermont College with her RN degree over ten years ago and has since served as both an Assistant Director of Nursing and a Director of Nursing. Tina achieved her first deficiency-free survey in 2017.

Ceylan Kucukaslan, MSW, LCSW
Social Services DirectorCeylan is the Social Services Director at HHS. She received her Master’s Degree in Social Work at Roberts Wesleyan College in Rochester, NY. She is a licensed Social Worker in New York and in Massachusetts. Ceylan started her career in mental health working with a broad age group and eventually transitioned to Long Term Care where she found her passion. As a Social Worker, she enjoys helping and advocating for residents and their families. She continues to make a difference in the lives of the residents and their families.

Khouloud Mandour
Human Resource ManagerMy name is Khouloud Mandour. I am a 2020 Bay Path honors graduate with a bachelor’s degree in Interior Design and Business Administration. My passion for HR started in 2019, when I was an intern at the Sheraton Monarch Place Hotel. I enjoyed engaging with employees on a daily basis and solving any problems they may have. This is where I knew that this is the route I actually want to take. I was eager to learn new tasks everyday and get all the experience I can. During my free time, I enjoy doing DIYs and bringing old things back to life, I enjoy reading and spending time with friends and family. I am excited to be joining the Genesis family!

Laura Ward, MOT, OTR/L
Director of RehabilitationLaura is a Director of Rehabilitation on the Heritage campus for Heritage Hall South. Her career began as an Occupational Therapist in 2006 at Heritage Hall West and has since transitioned to role of Director of Rehab. She has completed specialized training in dementia, seating and positioning, low vision and modalities. Laura’s passion is working in the skilled nursing setting helping individuals meet their goals and supporting clinical excellence with her team of therapists.

Great Team!
My dad has been at HHS for about 4 years now. The entire staff has been wonderful, not only in taking care of him, but also being available when mom and I have any questions or concerns. The administrator keeps us updated on any issues or changes, especially due to COVID-19, which is greatly appreciated. HHS has a great team!

It is never easy having to place loved ones in a Nursing Facility but Heritage Hall South has such a homey, comfortable atmosphere that you immediately know you made the right decision. I live out of state with two dear family members residing there and I can’t commend the staff enough for keeping me posted on any new developments that arise and addressing any concerns I may have. Love the convenience of On-site Dialysis
I live at HHS because of the convenience of the ON-SITE dialysis center. I don't have to get up as early in the morning or take a 30-minute ride back and forth from another facility. It's the best in the winter because I don't have to deal with going out in the cold. It's all right here!"
