Hackett Hill Center is a 70-bed skilled nursing facility now offering ACCELerate Skilled Nursing and Rehabilitation. ACCELerate is designed to help you get back home as quickly as possible after a hospital stay. Hackett Hill has earned the Skilled Nursing Facility Heart Failure Certification offered by the American Heart Association®, the world’s leading voluntary organization focused on heart and brain health. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. We accept Medicare, Medicaid, and most private insurances. Hackett Hill Center is an AHCA Bronze Quality Award Winner and a We Honor Veterans community partner!

Libby Haidemenos, MBA, LNHA
AdministratorLibby has been working for Genesis since 2008. She started her career with the company as a Dietary Aide. Since then, she has assumed roles of Resource Coordinator at the CareLine, Administrator in Training, and Assistant Administrator. Libby earned her Master of Business Administration degree with a Specialization in Healthcare Management from Southern New Hampshire University. She also holds a Graduate Certificate in Finance from Southern New Hampshire University. Libby has been licensed as a Nursing Home Administrator in New Hampshire and Massachusetts since 2013.

Dr. Ayobami Ojutalayo
Medical DirectorDr. Ayobami Ojutalayo is a Board Certified Physician in Family Medicine. Dr. Ojutalayo graduated with an honors degree in biology from the Montclair State University in New Jersey after which he proceeded to acquire a Doctorate degree from the Ross University School of Medicine in 2007. He completed his Family Medicine Residency at the prestigious Dartmouth Medical Residency Program in New Hampshire. During his residency training experience, he developed a passion for teaching other residents and was awarded the best resident teacher in his program. Dr. Ojutalayo has been with Genesis for the past five years.

Nancy Sydow, NP
Cardiac ConsultantNancy graduated form University of Massachusetts, Amherst with a Masters degree in Microbiology before going on to get her Masters of Science in Nursing from the University of Massachusetts, Worcester Medical School in 2008. She has been practicing as an acute care NP since that time and is currently Chief Nurse Practitioner for Merrimack Valley Cardiology. Nancy opened and runs the Heart Failure Clinic at Lowell General Hospital in Lowell, MA. Nancy has clinical interest in heart failure and AFIB. In her free time, Nancy enjoys spending time with her family and volunteering. Some of her favorite activities outside of work include skiing , running, and scuba diving

Katelyn Dumont, RN
Director of NursingKatelyn has been working in long term care since 2004 after graduating from Salem High School with her LNA license through the Health Occupations vocational program. She went to nursing school while working as a nursing assistant becoming a LPN and later completing a Registered Nurse Program at St. Joseph's Hospital in Nashua. Katelyn has worked various management positions in long term and short stay rehab nursing. Her focus is on providing quality, comprehensive individualized care with dignity and honesty. She is excited to be at Hackett Hill Center in this leadership role as well as working for Genesis where there is room for additional growth.

Austin Cole
Admission DirectorAustin graduated from Salve Regina University in 2021 with a B.S. in Healthcare Administration and Management. In 2022, he received his Master’s degree in Healthcare Administration and Management as well from Salve Regina University. Prior to joining Genesis and more specifically the team at Hackett Hill, he completed his Administrator in Training program at a skilled nursing facility in Newport, RI. When he is not at work, he enjoys playing rugby and golf.

Timothy Lunn, OT/L
Director of RehabilitationTim has been practicing OT since 2001 in the state of NH. Graduate from the University of New Hampshire and the University of Southern Maine, his experience has spanned a variety of settings and roles that has established his leadership foundation. Tim has experience working in outpatient, short term rehab/acute rehab at CMC, referral services, and long term care since 2005. Tim has been very involved in the OT community working for NHOTA, teaching at UNH in the Occupational Therapy program, and mentoring numerous students/employees. He has been noted by his peers in 2012 by NHOTA for a Service Award, by Genesis in 2010 for Clinical Innovation and Practice, 2011 for Business Excellence, 2012 for Customer Excellence and Clinical Excellence. He has also been noted by NHOTA as Practitioner of the year in 2015. He has a very strong passion for the Skilled rehab population and its purpose for those that require his teams services.

Bryan Fischer
PhysiatristBryan has been a PA for over 7 years with experience in Urgent Care and Neurosurgery. He obtained a degree in Nutrition & Wellness and is very passionate about health, exercise, nutrition, and ones overall well-being. While at Hackett Bryan will be working closely with the nursing and therapy team here to help support our patients progress in therapy to ensure a safe return home. Bryan is a US Army Infantry Veteran, and a NH native.
