Edmonson Center is an 86-bed skilled nursing facility located in beautiful downtown Brownsville, KY just minutes from Mammoth Cave National Park. Our center is comprised of 74 skilled nursing beds and 12 senior living beds and is made up of spacious private and semi-private rooms.

Chris Swihart
AdministratorChris has a degree in Healthcare Administration and has over 14 years of long-term care experience. Chris has been the Administrator at Edmonson Center since December 2014. Chris is compassionate and dedicated to every resident and family member. He states, "My goal is to improve the lives of our residents by providing quality care to each of them every day with kindness and compassion."

Dr. Pravin Avula
Medical DirectorAfter earning his medical degree in his native India, Dr. Avula pursued his Graduate’s degree in Microbiology and Immunology at Tulane University School of Medicine in New Orleans, LA. Dr. Avula completed his residency at the Coney Island Hospital in Brooklyn, NY before starting his private practice in Smiths Grove, KY. Dr. Avula is Board Certified in Internal Medicine and Sleep Medicine.

Lorie Wardlow, APRN
Nurse PractitionerLorie graduated from WKU with her Associates in Nursing in 1992 and later graduated with her Masters in Nursing from Vanderbilt University in 2004. Lorie is board certified as an Adult Nurse Practitioner with a focus in geriatrics. She has 29 years of experience in nursing with the last 17 years as a nurse practitioner. Her first 13 years as a NP were in primary care and the last 4 years have been in long-term care, where Lorie has found her true love. She has been at Edmonson Center since February 2017. Lorie was born and raised in Edmonson County and is very proud to serve the members of her community. She and her husband are lifelong community members where they are raising their 2 children. Lorie has a true passion for caring for others. She believes in caring for the patients as well as their families. It is her goal to always provide true quality care while also providing compassionate care to her patients.

Joseph Stewart
Director of NursingJoseph comes to us with a very diverse background and experience. After serving in the United States Air Force from 1992 to 1996 where he worked with B-2 Stealth Bombers, Joseph worked as a restaurant manager for many years. In 2012 Joseph made the decision to follow his passion and joined the Nursing Program at Western Kentucky University where he attained his ASN in April of 2014. His passion for nursing led him to further his nursing education by returning to WKU where he attained his BSN degree in 2017. Joseph joined the staff at Edmonson Center in July of 2018 and advanced to the Director of Nursing in February of 2020. Joseph is a natural here at Edmonson. Although he was not born in Edmonson County, he fits in perfectly and he has a great love of our community and the people that we serve.

Tara Montgomery, M.S. CCC-SLP
Director of RehabilitationTara graduated from Western Kentucky University with a masters in Communication Disorders in 2012. She has been working as a Speech Language Pathologist in long term care for 10 years. Tara has a passion for caring for others and achieving the highest quality of life possible. Tara resides in Edmonson county with her husband and young children. Outside of work, you will find Tara enjoying time with her family and intentionally celebrating life.
