Positive Outcome
Thank You
At a difficult time (in the best of circumstances), I commend the staff in skilled nursing and physical therapy at the Desert Willow facilities of the Village at Northrise. From intake to meals to dailies and discharge, all staff was caring, responsive, accommodating, and hard working. We owe particular thanks to Starla, the director of the nursing program, for her skills and empathy; Aide for her kind and funny approach to all her work; and to the entire PT/OT team for their positive effect on our family member. Honestly, I would shout out everyone if I could remember their names. An additional nice-to-have here: with rabies documentation, the dog could visit with us too. Though the hospital "placed" our family member here, we'd choose this facility again in a heartbeat.
So Kind
The lady behind the check-in came out and graciously helped me get set up with your new face scanner. (She knows me and that I don’t see well so she put my information in without me asking her - and I thanked her for it.) I went to visit my friend, your resident. Everything looked clean and pretty. While visiting my friend 2CNA’s came in to deliver meds. The second CNA was shadowing the first one as training for her employment there. Later the 2 CNA’s came in again to discuss the next days menu with my friend. Both times the CNA’s were very friendly and answered questions they were asked.
Very Caring
great place for therapy! admissions team and the administrator are very caring. highly recommend!!
Helpful and Caring
Admission staff is very helpful! They went out of their way to help me with my mom. They actually care about the patients. Welbrook May have private rooms but they don’t care about the actual patients. I will always be thankful for making the right choice! Highly recommend
Thank You
Our family member, post hospitalization, needed short term skilled nursing. At Desert Willows the staff was professional and attentive. The facility was clean. The kitchen staff tried to accommodate his dietary needs. The therapy was excellent!

“I am very thankful for all the staff here. I am so happy I am on my way to gaining my
independence back! This would not be possible without everyone who has helped me. I love the Village at Northrise.”
The Challenge
Ray Ashabaugh admitted to The Village at Northrise – Desert Willows from Memorial Medical Center after a very complicated hospitalization. Ray suffered from respiratory failure and required intubation during his stay. He was finally extubated on May 6, but continued with significant altered mental status and severe debilitation. Ray previously lived at home alone and was independent with all tasks. Upon admission to our facility, Ray required Max assist for bed mobility and transfers, was unable to stand, and required assistance for all his ADLs (Activities of Daily Living).
The Treatment
Ray participated with Physical and Occupational therapy during his stay. He continuously worked on improving his strength, transfers, and ambulation. By his 2nd week at the facility, Ray was back to walking 75ft with a front wheel walker and only required supervision for his ADLs. Therapy continued to focus on making Ray as independent as possible, while improving his balance and safety.
The Outcome
Ray is now Independent with all his ADLs and is back to walking over 200ft with no assistive device! He will be returning to the community on 6/11/21. He expresses his sincere thanks to the entire therapy team.
