Susan Dufresne
AdministratorSusan came to us from a rewarding career as an Licensed Nursing Home Administrator in sunny Florida. She was born in Providence, Rhode Island and moved to Florida over 10 years ago to get away from New England's harsh winters. She returned to Massachusetts to be closer to family. She is a graduate of Rhode Island College and Franklin Pierce Law Center in Concord, New Hampshire. She was also a Preceptor/Mentor for new Administrators and looks forward to sharing her years of professional experience with others in the field. She is a member of Toastmasters International, Eastern Star, and has volunteered for New Hope of Bristol County.
Fabian Delgado, MD, FACP, CMD
Medical DirectorDr. Delgado fluent in both English and Spanish, graduated with his medical degree from the Catholic University School of Medicine in Guayaquil, Ecuador. A Board Certified Internist, he completed his residency in Internal Medicine at the University of Kansas Medical Center and successfully completed a Fellowship for Healthcare Leaders at the University of California, San Francisco. He is a Fellow of the American College of Physicians, and has extensive experience in medically complex patients in all settings: Acute care, Long-Term Acute Care, Post-acute care and Hospice care. Additionally, Dr. Delgado is a Certified Medical Director by The American Board of Post-Acute and Long-Term Care Medicine and is passionate about quality improvement in post-acute care.
Dr. Delgado has served in other leaderships roles as the Medical Director of multiple post-acute care facilities in Massachusetts and Rhode Island and has supervised medical residents at TUFTS Medical Center, Transitional Year Residency Program at Brockton Hospital. Currently affiliated with Vantage Healthcare, an experienced, high-performing physician practice for seniors.
As the Medical Director of Warren Skilled Nursing & Rehab Center since 2020, Dr. Delgado compassionately directs and delivers high-quality healthcare within our family oriented team setting. And whenever time allows, he enjoys travelling, dining, soccer, American football, baseball and mixed martial arts.
Jennifer St. Angelo
Director of NursingJennifer St. Angelo, our Director of Nursing loves being a nurse. A life long resident of the East Bay, Warren Bristol area, Jen started her nursing career after graduating from high school as a certified nursing assistant. She enrolled while raising her family and graduated from the Community College of Rhode Island with her RN degree. Having worked as a clinical floor nurse in a skilled nursing facility for a number of years, Jen understands what it is like to work the floors. She was promoted to the Nurse Educator and Infection Control Practitioner in 2019 and was certified in wound care and infection control. Warren Skilled Nursing & Rehabilitation Center promoted Jennifer to our Director of Nursing in August 2022. As the center's nursing leadership Jen makes a difference in the lives she touches and is emotionally rewarded by the sense of purpose her role plays in the patients we care for. Her passion is evident in all she does.
Hannah Allain, COTA/L
Rehabilitation Program DirectorHanna joined the Warren Center team in 2018. She earned her degree from Bristol Community College and completed her internship at Butler Hospital, Geriatric Unit. Hanna is currently enrolled in the University of Massachusetts at Dartmouth's Health Care Management Program.
Sokunthea “Thea” Michaloski
Social Services DirectorSokunthea “Thea” (pronounced like “Tia”) Michaloski is our Director of Social Services. Thea, a 1999 graduate of Rhode Island College has worked as a social worker in various positions and has extensive experience working with patients with mental health and mental illnesses, including Schizophrenia, Alzheimer’s and Dementia. She most recently worked as a mental health counselor at a hospital in Metro West, Massachusetts.
Thea is a native of Cambodia and is fluent in Cambodian. Her experiences living in refugee camps as a child helped led her to want to become a social worker and give back to the community. Thea loves being a mom; her 19 year-old is a junior in college and her 13 year-old is in seventh grade. She even has a cat. In her free time, Thea loves to cooking, collecting cookbooks, gardening, paddle-boarding, surfing and working out at the gym.

"I spent over a month at Warren Skilled Nursing & Rehabilitation Center from November 30, 2022 until January 12, 2023. I was admitted after a 14 day stay at Rhode Island Hospital with a staph infection. I received daily intravenous 3 hour antibiotic treatments while at Warren Center. From the very first day, I could not have been happier with my treatment and care. The one special request I had was to be able to walk outdoor for my healing. It was granted. I am from California mountain area at 10,000 feet elevation. I am a hiker, skier and snow boarder, used to vigorous exercise at high elevation. The outside walking really helped me. Everyone was attentive to my needs promptly. The three meals a day were great and I asked for larger portions because my appetite increased and it happened. Thank you everyone."

Privacy and safety
A great job is being done to accommodate both our father and us as the visitors. Privacy is given to us during the visit while the staff is still able to assure all safety measures are kept. The visiting area outdoors was as cool as it could be during the heat wave and we enjoyed our visit. Great job!!!

Excellent rehab and good meals
My experience at the Warren Skilled Nursing & Rehab Center has been a very positive one. Meals have been overall very good. Nursing and rehab has been excellent. Staff members were very understanding, caring and compassionate.

Wonderful staff, PT and activities
Hurting my ankle has not been bad for me. The staff at Warren Skilled Nursing and Rehab Center have been wonderful. I enjoy the activities. Physical Therapy has been very hard but worth the work out. Occupational therapy is excellent too. The food is good, hot and nice tasting. The nurses are on their toes to answer the light for all of my needs.

The entire staff are top notch. The food is good, Always on time with my medications. The entire building is just fantastic. Good activities. Outside courtyard is just so nice. The staff is very attentive for whatever you need. I would highly recommend this facility for anyone who needs rehab and care. And everyone is very big on safety. On a scale from 1 to 10 Warren Center is a 10.

Wonderful experience
What a wonderful experience at the Warren Skilled Rehab Center. The administration, nurses and staff did everything to make me feel at home. Overall, the staff led to my speedy recovery from my open heart surgery. I had 2 valves replaced and corrected an atrial fibrillation problem.

Treat you like family
Upon admission I had a below the knee amputation and needed therapy to heal and a prosthetic leg. The rehab therapy was fantastic. After spending time in a wheelchair, they were able to get me to walk with a walker and now I am able to go a good distance without a walker. My care was incredible. They work as a team to meet my needs, activities was enjoyable, food is good and when I want something else there is always a choice. They treat you like family. I would recommend Warren Skilled Nursing & Rebab Center to anyone.

Highly recommend
The Warren Skilled Nursing Center is an excellent organization. The staff is knowledgeable, kind hardworking and responsive. Management also has the commitment to top quality patient care. The meals are fresh and tasty. Chef Jackie aims to provide residents with health and delicious food. The activities director is cheerful and creative and brings happiness to the residents she serves. The supervising physician, Dr. Fabian Delgado, is dedicated to giving the residents wonderful medical care. He also gladly takes the time to speak to patients and their families in order to give updates on the patient's progress. Overall I highly recommend the Warren Skilled Nursing Center.
