The Elms Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care. We accept Medicare, Medicaid, and most private insurances. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. Dr. Ayobami Ojutalayo
Medical DirectorDr. Ojutalayo is a Board Certified Physician in Family Medicine. Dr. Ojutalayo graduated with an honors degree in biology from the Montclair State University in New Jersey after which he proceeded to acquire a Doctorate degree from the Ross University School of Medicine in 2007. He completed his Family Medicine Residency at the prestigious Dartmouth Medical Residency Program in New Hampshire. During his residency training experience, he developed a passion for teaching other residents and was awarded the best resident teacher in his program. Dr. Ojutalayo has been with Genesis for the past five years.

Jennifer McFadden, NP
Nurse PractitionerJennifer is a board certified nurse practitioner who began working for Genesis in 2017. In 2013, Jennifer earned her BSN, and then continued to her education to receive her MSN, APRN and FNP-C in 2016, both at Chamberlain College of Nursing. Prior to working for Genesis, she spent the majority of her nursing career as an RN at a local hospital. She worked in the medsurg, and surgical trauma units. She is now the NP at Crestwood and The Elms in Milford, and enjoys caring for all residents. Jennifer grew up passionate about taking care of others, and that translates very strongly into her performance at work. Her favorite part of each day is seeing and spending time with the residents. The most rewarding part of her job is getting to know the residents and being able make a positive impact in their lives.

Amanda Peabody
Director of RehabilitationAmanda went to school at Franklin Pierce University for both a Bachelors in Biology and then a Doctorate in Physical Therapy. She graduated in 2012 and have been working for Genesis ever since. She has also worked in home care but has preferred the SNF setting. I chose PT as a career as a sophomore in high school. She went to a technical high school in Massachusetts called Montachusett Regional Vocational Technical High School. It was there that she went through medical assisting training on shop weeks and the clinical instructor gave them an assignment to research the medical field for other areas we may be interested in. In her research she learned about Physical Therapy and this was really interesting to her. Initially, she only wanted to be a PTA but was encouraged and given support by a clinical instructor from a co-op she completed in an outpatient PT rehab center to pursue her DPT. The rest is history! She enjoyed working with this population for both the challenges and the rewards I see everyday.

Emily Morehouse
Admission/Social Service DirectorEmily joined the team at The Elms Center in January of 2023 but previously worked at The Elms at the end of 2020 while covering a medical leave. She is happy to be back at The Elms Center again. She has been a social worker for the past 7 years and enjoys working with the elderly. She is excited to learn the admission role at the facility. When not at work she enjoys spending time with her sons.
Committed and caring
From a LTC resident's spouse: To The Staff at The Elms; A fun day was had by all at the Hoe Down celebration held Thursday. Thanks for putting together such an exciting event. Even the rain held off. It takes a committed and caring group to pull off such a wonderful experience for all. Adding Don Smith made it just that much more fun! Our hats off to the staff!

The nursing staff made my stay more pleasurable. Alison, Maria, and Barbara were very encouraging. I knew they cared about me.
