The Elms is a 52 bed facility located in the town of Milford, NH near the historic oval. We strive to offer a home-like atmosphere to help comfort our residents and patients. In the summer time the local parades can be seen by the front area of our facility and the town fireworks can be seen from the back parking lot, which overlooks the towns beautiful Keyes Park.

Matthew Simoneau
Center Executive DirectorMatthew Simoneau is the CED at The Elms Center. A little bit of information about me, I have a bachelors in Pre-Med Health and a Master’s degree in Healthcare Administration. I was raised in long- term care health as most of my working career has been spent in long-term care buildings. I have a wife and 2 year-old daughter who loves to make my days crazy, but I would never trade it for anything else. We hope your time at The Elms Center is enjoyable, as we have a great team that works hard to make sure you feel supported through this busy part of life.

Dr. Joseph De Vera
Medical DirectorDr. De Vera is a Board Certified Internal medicine physician. He has seventeen years of experience as a Hospitalist that includes Catholic Medical Center and Monadnock Community Hospital. That experience provides Pheasant Wood Center residents a wealth of clinical expertise. Dr. De Vera also has eight years of outpatient primary care and wound care at both DHMC/Catholic Medical Center and Monadnock Internal medicine. As a member of the Genesis Physician Services he is also the Medical Director at the Pheasant Wood in Peterbourogh, NH. Dr. De Vera works very closely with our residents, their families and our staff

Center Nurse ExecutiveBrandee is the Center Nurse Executive at The Elms Center.

Anna Lyon
Senior Admissions DirectorAnna graduated from Southern New Hampshire University in 2016 with a Bachelor of Science in Healthcare Management. She has enjoyed working in the healthcare field for several years now. Anna previously worked in a dual role at The Elms as the Director of Admissions and Social Services for 2 years. She was promoted to Senior Admissions Director for both Crestwood and The Elms in February 2021.
Amanda Peabody
Director of RehabilitationAmanda went to school at Franklin Pierce University for both a Bachelors in Biology and then a Doctorate in Physical Therapy. She graduated in 2012 and have been working for Genesis ever since. She has also worked in home care but has preferred the SNF setting. I chose PT as a career as a sophomore in high school. She went to a technical high school in Massachusetts called Montachusett Regional Vocational Technical High School. It was there that she went through medical assisting training on shop weeks and the clinical instructor gave them an assignment to research the medical field for other areas we may be interested in. In her research she learned about Physical Therapy and this was really interesting to her. Initially, she only wanted to be a PTA but was encouraged and given support by a clinical instructor from a co-op she completed in an outpatient PT rehab center to pursue her DPT. The rest is history! She enjoyed working with this population for both the challenges and the rewards I see everyday.
Committed and caring
From a LTC resident's spouse: To The Staff at The Elms; A fun day was had by all at the Hoe Down celebration held Thursday. Thanks for putting together such an exciting event. Even the rain held off. It takes a committed and caring group to pull off such a wonderful experience for all. Adding Don Smith made it just that much more fun! Our hats off to the staff!
