Spa Creek Center is located in a suburban residential neighborhood just outside Annapolis, Maryland's historic capital district. Built in 1993, it is an attractive, modern two-story building set away from the street in a serene wooded landscape. Spa Creek provides LongTerm Care and ShortStay Care including Rehabilitation in our Transitional Care and Progression Orthopedic Units. In 2009, Spa Creek was the 1st Maryland Skilled Nursing Facility to win the AHCA Silver Award. Within the Annapolis community, our Center is regarded as a highly desirable destination for both LongTerm nursing care and ShortStay Rehabilitation.

Samantha Smith, NHA
Center Executive DirectorSamantha has worked for Genesis HealthCare since 1998 and became a Licensed Administrator in October 2013. She has over 20 years of leadership experience and is a graduate of University of Maryland University College. She was previously the Administrator at Caton Manor and Hammonds Lane Center. Samantha is dedicated to the patients, residents, families, and staff members and thrives to provide an environment with a strong focus on customer service and quality care. She shares her company’s mission in improving every life she touches.

Ellen Cassidy
Medical DirectorHi, I am Ellen Cassidy. I am a board-certified family practice physician who has practiced medicine for over twenty years. For the past eight years, I have pursued a particular interest in geriatric medicine, including completing rigorous additional training to become a certified medical director of skilled nursing facilities. I currently serve as medical director and attending physician at Genesis Spa Creek Center in Annapolis, and medical director at Corsica Hills Center in Centreville.
In these roles, I have been proud to see first-hand the dedication Genesis HealthCare has shown to patient and staff safety during the Coronavirus pandemic. As a nationwide leader providing both short-term rehabilitative and long-term care, Genesis has been able to secure personal protective equipment (PPE) and contract with labs for Covid-19 virus testing for staff and patients. Moreover, the nurses, GNA's, therapists, and all of the other ancillary front-line workers have never wavered in their commitment to our patients!
Throughout the pandemic, Genesis continues to follow strict CDC guidelines for patient safety. In fact, in many cases, it has even exceeded those promulgated by the CDC. We at Spa Creek and Corsica Hills have been diligent in practicing infection control to protect our patients.
In closing, I am proud to serve as a medical director working with staff who have bravely served and continue to serve our patients through a once-in-a lifetime pandemic.

Carrie Dana-Evans, CRNP
Nurse PractitionerCarrie attended the University of Virginia, graduating in 2006 with her BS in Nursing. She worked for two years at Georgetown University Hospital on the transplant unit before moving to Annapolis where she worked on the Oncology Unit at Anne Arundel Medical Center. She went back to school in 2010 for her MS degree in Adult/Geriatrics and upon graduating was certified as an Adult Nurse Practitioner. She started working for Adfinitas Health at Genesis Spa Creek in April of 2012. She has been providing care for adult and geriatric patients at Genesis Spa Creek Center for over eight years. We are so glad to have her on our TEAM!

Vanessa Demetrius-Clarke, RN
Center Nurse ExecutiveVanessa has been a Registered Nurse since 1997 and has worked in many settings and in many capacities over this period. She has hospital, Long Term Care, Assisted Living and Home care experience, and has worked as charge nurse, supervisor, unit manager, nurse educator, infection preventionist and director of nursing in the skilled nursing home setting. Vanessa believes it is a privilege and joy to work as a nurse and fulfil her only occupational dream but also believes it is her responsibility to ensure that those entrusted in her care receive the best quality care.

Neema Simms
Director of Marketing & AdmissionsNeema has been with Genesis HealthCare Spa Creek Center since 1994. She has been awarded Employee of the Year in three different positions. Since January 2010 serving as the Admissions Director. Genesis Core Values along with providing quality care and excellent customer service for her patients and community is her creed.

Jaimie Nelson
Director of RehabilitationJaimie Nelson is a graduate of Towson University with a Master’s Degree in Occupational Therapy and has been an Occupational therapist for over 12 years. She has worked for Genesis her entire career and has been in a Director’s role since 2010. Jaimie strives hard to work with her rehab team to facilitate positivity and growth in all the patients they touch.

Walking a lot better!
“I’m able to go up and down stairs so much better now, compared to before. Also I’m walking a lot better with the walker”. Patient is able to go up and down 4 stairs with bilateral rails and supervision, walk 180-200ft with rolling walker and supervision, bed mobility and transfer on her own.

Impressed and grateful
When my mom got to Spa Creek she was unable to stand, walk or transfer without a lot of assistance. She could barely facilitate any volitional movement of the tight arm and leg, but after tremendous hard work on her part, in conjunction with the strengthening, functional mobility tasks she performed with PT and OT team she was progressed vastly. She now can transfer and perform bed mobility with supervision, she can walk using a wheeled walker up to 130 feet with limited to no assistance and has even began performing stairs. Alverta has said on multiple occasions that she is very impressed with how far she has come and how grateful she is for her rehab team!

So thankful!
When Ms. Conley came into us this time she was max assist/ dependent for transfers, gait, and bed mobility. This was my third time working with her at Spa Creek. On this admission I told her son whom I have gotten to know real well from her previous stays that I was not sure if she would get back to her baseline. Not only was she able to return to her baseline she has improved. Able to transfer with just having someone by for support and walking up to 70ft with a walker. Ms. Conley reported, “I am so thankful for all you have done for me. Thank you for sticking with me and getting me back to being able to walk as well as I am so I can go home.

I feel fortunate
After first meeting a cheerful nurse Anslem, the mood was set for a positive post-op experience at Spa Creek. The employees are caring and helpful. The facility is clean and comfortable. The food is surprisingly good! My therapy team of Yasmeen and Kim kept me challenged while keeping my pain under control and the routine varied to keep the boredom at bay. I feel fortunate to have ended up at Spa Creek. I will miss everyone’s smiling faces! Jamie C. New Positive insights...
Kenneth M. came to Spa Creek center as a new above the knee amputation who had been through quite a medical journey. Initially he was a bit resistant to completing rehab treatment due to his high level of frustration with the medical process he had been through. However, once he built a rapport with his therapists (Frank/Ebone’), he was able to make the most out of his experience here at Spa Creek and became a success story. With the help of our rehab team, Kenneth not only has a positive mindset with regard to the journey he has ahead, for getting a prosthetic leg, he also gained skills to maximize his independence to be able to discharge back home. Mr. M. has not only gained new positive insights but also actively participates in his therapy sessions everyday and even encourages his roommate to complete their therapy as well because “it will get you better and moving again, like me”. Mr. M. will discharge to home from the facility on May 26, 2020, functioning at an independent level, where he will be fully equipped to start his therapy journey with in home therapy for his new prosthetic leg.
What a gem!
I would like to recognize Beth Novak! She has been amazing. My mom came to Spa Creek about a month ago from the hospital with a broken pelvic and fractured sacrum. She was in such bad shape I didn’t think she would make it. But, here we are and she is leaving Friday and headed to an Assisted Living in Easton MD. Obviously the PT and OT and nurses did their part but Beth was always there for me. She walked me through everything. From what Medicare and Insurance would or wouldn’t pay for. My options for finding an Assisted Living for my mom. How to prepare my mom for the realization that she wasn’t going to be able to go home. Perhaps the most important thing. She coordinated FaceTime calls with my mom every week and followed up with me with an update on her progress. Lastly, she had a zoom call today with myself, my mom and the new facility to do the assessment and finalize moms move for Friday. I couldn’t have done it without her help and guidance along the way. And she let me cry when I needed too. What a gem! And, please let the PT and OT folks know I am very grateful to them. My mom is 100% better than I ever expected.

They take care of me real good here!
When we as a family had to make a decision to admit Aunt Henrietta to your care we knew it was for the very best because we couldn’t give her the care she needed; but when this Coronavirus broke out we must confess that we were very scared especially hearing about other health facilities. The one person who was not afraid was our beloved Aunt Henrietta. She is and has been a woman of faith for the past 95 years. Specifically speaking (and I can speak for the entire family) the frequent testing is absolutely crucial and beneficial. Frequent testing of both the residents and the staff keeps everyone abreast of this stressful situation and above all it gives peace of mind. The staff always keeps us informed in a very timely manner and the ZOOM meetings are truly a God send. Of course, we sorely miss the in-house visitation as it once was but we know it is impossible right now. The safety of Aunt Henrietta and all the residents come first. We are comforted with the outside drive by visitation and we are blessed to see Aunt Henrietta, even for a little while and she is thrilled to see us. The family who cannot make it to the drive by visitations make use of FACETIME, video calls and phone calls. Randi Sue and Latif are VERY, VERY accommodating. There was one time upon leaving after a visit with Aunt Henrietta, I asked her: “Aunt Henrietta … are you lonely?” She responded: “No sweetie! They take care of me real good here!”
