Sarah S. Brayton Center is located at the north end of Fall River, nestled into a quiet suburban area with the convenience of the city minutes away. We offer a specialized Transitional Care Unit for patients needing Rehabilitative Care or continued Medical Management after a hospital stay. Our professional, dedicated and compassionate staff want to assist you in the development of your individualized treatment program. Sarah S. Brayton's on-site nurse practitioners and attending physicians work together with our staff to be sure you receive the best medical care possible as we work to get you back to your home quickly, and at the most independent and safest level as possible.

Sandy Sarza, BA, NHA
Center Executive DirectorSandy joined the Genesis team as the Center Executive Director at Sarah S. Brayton Nursing Care Center in 2016. Sandy is a graduate of Stonehill College with a Bachelor's degree in Health Care Administration. Sandy is a Licensed Nursing Home Administrator in the states of Massachusetts and Rhode Island. She has worked in the health care industry since 1981. Sandy has worked for several large corporations in skilled nursing care at various levels. Sandy is also a Certified Activity Consultant, an Instructor for the Modular Education Program for Activity Professionals, a Certified Dementia Practitioner and a Certified Alzheimer’s Disease and Dementia Care Trainer.

Dr. Fabian Delgado
Medical DirectorMy name is Dr. Fabian Delgado and I have been a practicing physician for over 19 years in Massachusetts. With the COVID-19 Pandemic continuing in many areas throughout the United States, we have put protocols and precautions in place to keep our admitting patients and our long term residents as safe as possible. First, all of our patients are admitted to a dedicated Admission Observation Unit for 14 days where they are closely monitored and tested for COVID-19. Second, Employees have been trained in the use of Personal Protective Equipment (PPE) and infection control guidelines. I encourage everyone to wear the appropriate PPE, because there is exposure no matter where you go. Third, all employees are tested for Covid-19 on a regular basis and are screened for temperature and symptoms every shift, and only work if they pass this clinical screening process. Everyone gets screened daily, and it is the best way to keep yourself and everyone around you safe. By following guidance from the MA DPH and CDC, our goal at Sarah Brayton is to provide high quality rehabilitation and medical care in a Covid free environment.

Patti Copeland
Center Nurse ExecutivePatti started her healthcare career as a CNA. She fell in love with caring for patients and their families and knew she found her calling. After a few years, she obtained her Licensed Practical Nursing (LPN) Diploma at Diman Regional in Fall River. While working as a LPN, Patti attended Bristol Community College and earned her degree in Nursing, becoming a Registered Nurse in both Rhode Island and Massachusetts in addition to majoring in social work at Bridgewater State College. Patti began her career with Genesis in 2003 as a staff nurse where her hard work was recognized, as she was promoted to TCU Manager, Assistant Director of Nurses and Nurse Practice Educator. In 2016, Patti was promoted to Center Nurse Executive for the Milford Center, where she improved the quality of the center for both staff and residents by utilizing her years of experience and education. When the CNE position became available at Sarah Brayton, Patti jumped at the chance to come back to her hometown so she could be closer to her family and to work with the community where it all began.

Angel Silva
Admissions DirectorAngel has been the Director of Admission at Sarah Brayton since 2016. She has over 30 years of experience working in the healthcare field. Angel is very passionate about long term care and is committed to building a strong and meaningful relationship with the residents and their families. She is focused on customer excellence and marketing.

Shelly McCann, BS COTA/L
Director of RehabilitationShelly has worked for Genesis for 15 years and joined the Sarah Brayton team in February 2019. She is a lifelong resident of Fall River and graduate of BMC Durfee HS. Shelly earned her Associate of Science in Occupational Therapy from Bristol Community College in 2001 and Bachelor of Science in Health Services from University of Massachusetts Dartmouth in 2015. She is a licensed Occupational Therapy Assistant in Massachusetts and Rhode Island. She brings 20 years of Occupational Therapy and rehabilitation in Skilled Nursing experience to our team. She is committed to providing quality, effective rehabilitation to each and every resident in our care.
