Putnam Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care. We accept Medicare, Medicaid, and most private insurances. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from illness or injury. Julie Bishop
AdministratorJulie Bishop, Administrator
Julie started her career with Genesis in 2019. She has served in many capacities while being with Genesis - most recently as a Senior Administrator. She graduated from Glenville State College in 2014 with a Bachelor Degree in Business Management. She went on to obtain a Master of Business Administration with a focus in healthcare management & graduated in 2019. Julie strives to promote a teamwork environment to ensure each resident receives the highest quality of care. Julie is dedicated to promoting a positive environment for our residents & staff to have the best experience at the Putnam Center.
Katlen Gibson
Director of NursingKatlen Gibson, Director of Nursing
“I have been in the healthcare industry since 2011. As a former CNA I took my education further and became an LPN. I started my Career with Genesis Healthcare in 2015 as a LPN. I proceeded to obtain my BSN which was completed at Rio Grande university in Ohio. My ultimate goal for furthering my education is to help make a positive impact on health care. Taking care of people is something I love to do. I was born in NC and raised in Poca WV. I am married and have three beautiful children. I have had experience in many different fields such as pediatrics, med surg, cardiac, home health, ICU, psych, and LTC.”

A graduate of Poca High School and Rio Grande University, I chose a career in healthcare to make a difference!! Sara Parsons
Rehab/Specialty Program DirectorSara received her Master’s Degree in Physical Therapy from West Virginia University. She has over 10 years of experience in skilled nursing. Sara became the Program Manager at Putnam in March 2006. She takes pride in being a patient advocate and helping all patients reach and maintain their highest functional level.
Michelle Weatherholt
Director of Marketing and AdmissionsMichelle began her career at Putnam Center in 1993 as a CNA. She then accepted the Medical Records position, where she remained for several years. During this time, she also managed Staffing, Central Supply and assisted with Admissions and Marketing.
Michelle next accepted the position of Guest Services Director, where she was able to combine her love for our residents with her love for our Veterans. She built our We Honor Veterans program and earned Putnam Center their Level 4 rating. Michelle was able to share her passion for providing great guest service within our network, building the program and training new staff to their roles.
In 2023, Michelle accepted the Director of Marketing and Admissions position. This role has enabled her to remain involved in the lives of our current residents, while also being able to help new residents and their families find their place here at Putnam Center.
Michelle has been married to her husband, Brian, for 28 years. Together, they have two amazing children and a wonderful network of extended family and friends.
Tracy Adams-Rayburn
Guest Services DirectorMy name is Tracy Adams-Rayburn, Guest Services Director for the center. I began my career here at Putnam Center as a Dietary Aide in 1997. Since that time, I have worked in Recreation as an Activity Assistant and served as Recreation Director for the last 13 years. I accepted the Guests Services position in September of 2023. My purpose is to work closely with you and your loved ones to ensure that you are comfortable and content as possible while you are with us. Please do not hesitate to come to me with any concern or requests that you may have. I will help you to the best of my ability.
I am married and have 2 sons and 7 grandchildren. Crafting with my grandchildren is my most enjoyable hobby away from my work.
Kristen Shamblin
Nurse PractitionerKristen Shamblin is our Nurse Practitioner and joined Putnam Center in 2011. She graduated from Marshall University in 2003 and 2008 with a Bachelor of Science in Nursing and Master of Science in Nursing with Family Nurse Practitioner emphasis. She is a Board Certified Nurse Practitioner with over 10 years of long term care experience. She also has additional education in dementia, geriatrics, palliative care and wound care.
