Milford Center is a spacious 135 -bed Skilled Nursing & Rehabilitation facility offering short-term rehabilitation as well as extended and long-term care. We are located minutes from Interstate 495 with easy access to Boston, Worcester and Providence. The center is located in a peaceful and serene wooded area but convenient to the activities and services available in thriving community.
Our renovations completed in 2010 provide gracious interior areas, expanded therapy space and attractive dining options. Milford Center will offer you or your loved one exceptional care with individual treatment plans developed by physicians, nursing and therapy, which is offered 7 days a week.

Julius Tabe, MD
Medical DirectorJulius graduated Medical School from the University of Yaounde School of Medicine, located in Yaounde, Cameroon. His residency was completed at Dartmouth Hitchcock Family Medicine Residency at Concord Hospital, in Concord, New Hampshire. He is Board Certified with the American Board of Family Medicine and is licensed within the states of Massachusetts, New Hampshire, Connecticut, and Texas. Dr. Tabe is also affiliated with Lowell General Hospital, in Lowell, Massachusetts. Dr. Tabe always arrives with a smile on his face. He is very attentive to the needs and health and well-being of the residents at Milford Center. He strives to provide quality medical care.

Director of NursingJen is the Director of Nursing at Milford Center.
Jane Asembo, BSN, RN
Infection PreventionistJane received her Bachelors degree in nursing from Grand Canyon University. She has been working as a Nurse since 2008. She has been working with Genesis Milford Center since August 2020. She has also held the position of Director of Nursing at Genesis Milford before returning to prior position of Infection Preventionist. Prior experience include, Director of Nursing, Assistant Director of Nursing, Staff Development, Wound Nurse, and Infection Preventionist,. Jane has worked in Pediatrics, Geriatrics, and Psych settings to provide compassionate care throughout her years of service
Judy LeClaire
Social WorkerJudy Graduated from Western New England College with her degree in Social Work. She has been a Licensed Social Worker (LSW) since 1998. The majority of her career has been serving the Geriatric population in the Long Term Care setting. She is a Certified Dementia Practitioner and a Jewell Palliative Care Scholar. She joined Genesis Healthcare in July of 2022 as a Social Worker on the Cedar Unit assisting with short term rehab discharge planning. When not at work, she enjoys spending time with her children.
Congrats!
Mr. Reed spent 25 days at Milford Center. He arrived without the ability to sit without support or walk. He worked hard with our therapy department and left able to walk with his walker. Congrats to him!
