AGAWAM, MA – (May 28, 2019) – Heritage Woods, a senior living community affiliated with Genesis HealthCare, today announced that it was chosen to participate in the Alzheimer’s Association’s second phase of the Alzheimer’s and Dementia Care ECHO Program.  The program is aimed at enhancing the care for people living with Alzheimer’s and other dementias in assisted living communities.

Project ECHO® (Extension for Community Healthcare Outcomes) uses video conferencing technology to connect with dementia care experts from assisted living communities across the United States.  Experts come together every other week for six months to learn, provide feedback, ask questions and share experiences.  Project ECHO originated in New Mexico in 2003 to train primary care providers in rural communities to treat patients with Hepatitis C. The goal was to provide guidance and support so that clinicians could treat their own patients who are experiencing complex conditions, while reducing the heavy workload on specialists. It is rooted in case-based learning that encourages collaboration, continual learning and applied practice. Since its inception, the model has been replicated in 35 countries to cover over 100 diseases and conditions. The Alzheimer’s Association is one of the first to utilize the model for long-term and community-based care to address dementia.

Similar to the pilot phase of the Alzheimer’s and Dementia Care ECHO Program, which concluded this past February, the second phase will consist of a six-month program which will explore topics within the Alzheimer’s Association Dementia Care Practice Recommendations, released in 2018. The recommendations, developed by dementia care experts, define quality care across all care settings and throughout the disease course. The series will dive deep into the recommendation areas, using real-world cases to promote learning. Participants will have resources available to them throughout the series and will receive a certificate upon completion.
