Elm Wood Center, Skilled Nursing and Rehabilitation, provides both LongTerm Care and ShortStay Rehabilitation. We offer Skilled Nursing, Palliative Care, Hospice Care, Respite Stays, Medical and Rehabilitative Care for patients and residents. Elm Wood Center provides daily therapy services including Speech Therapy, Occupational Therapy and Physical Therapy along with 24-hour nursing care. Elm Wood has a spacious dining room offering restaurant-style dining and patients and residents are offered a choice for meals, including an always available menu.

Center Executive DirectorI was raised in the Long-term care field as my mother has been a long-term care nurse her whole life. I volunteered as a teen in the facility she worked in and worked my way up to Medical Records and Supplies Director within the facility. I have always enjoyed the geriatric field and even went to college pre-med in hopes of becoming a geriatric physician. However, I found that I enjoyed the business aspect of healthcare and pursued the career of Executive Director. I am going for my Masters of Healthcare Administration and have completed my Bachelors of Science. Also, I am married to a wonderful woman with an almost two year old daughter that is half angel and half destroyer of everything she touches.

Director of AdmissionsTonya started long term Care in August of 2002 where she fell in love with the industry. She began working for Genesis HealthCare in 2011 as a Rehab Office Coordinator in a larger center in Vermont. She moved to New Hampshire eight years ago. She has recently transitioned to Elm Wood Center as our Director of Admissions, which is closer to home and provides her the opportunity to serve those within the community she resides in. Tonya believes in a team approach and that each member plays a vital role in the health, wellbeing and happiness of the residents we serve.

Total Knee Replacement Success Story
Maria had both her knees replaced on December 14th. After her stay in the hospital, Maria came to Elm Wood Center for continued therapy because she needed help with just about everything including her Activities of Daily Living. Maria was challenged by her level of pain, making it difficult for her to stand or ambulate. After 8 weeks of intense therapy, Maria had progressed by leaps and bounds. She walked the halls on her own before she went into the rehab room for therapy. She was able to do her ADL’s independently, and she was seen making cookies in the rehab kitchen. Maria was able to successfully return home on February 1st and after checking in with her, she has been doing well at home since leaving Elm Wood Center.
