We understand the huge toll that separation has taken on our residents and families. We are excited to announce that our Genesis-affiliated Centers will now allow indoor visitation at all times and for all residents (regardless of vaccination status), with the exception of the three scenarios listed below. CMS, the federal agency that regulates nursing centers, has updated its visitation guidance due to high resident vaccination rates and a drop in COVID-19 cases around the country. read more

Elms Haven Center is a skilled nursing facility offering post-hospital, short-term rehabilitation as well as long-term and respite care services. We also have a specialized transitional care unit for patients transitioning from hospital to home and a Memory Support unit for residents with Alzheimer's or dementia. We accept Medicare, Medicaid, and most private insurances. Elms Haven Center is an AHCA Bronze Quality Award winner and WE HONOR VETERANS partner!

Donald Criger
Center Executive DirectorDon directs the ship as our Center Executive Director. He has over 29 years of experience in the healthcare field and comes to Elms Haven from previous Genesis Communities in Alabama and Georgia. He is a Licensed Nursing Home Administrator with dual degrees in Hospital and Health Care Administration and Accounting. Don believes in looking at the big picture and motivates staff towards our common goal of providing the best health care possible. He has a fondness for the geriatric community and is very proud to show off our beautiful, spacious remodeled facility. Don enjoys participating in competition barbequing, reading, and traveling.

Malcolm Fraser, MD, CMD
Medical DirectorMalcolm Fraser, MD, CMD has been a Certified Medical Director for over 20 years. Since joining the Elms Haven Team in 2018, he has helped the facility by initiating Monthly Meetings (Virtual since March 2020) with the Attending Physicians and Mid Level Providers (Nurse Practitioners and Physician Assistants) and also helps Elms Haven keep up to date with the latest COVID related regulations by attending Monthly Meetings with the Colorado Department of Public Health. He also participates in twice a month nationwide Genesis Physician Services Zoom Meetings so that he can also bring a nationwide perspective to the Care provided at Elms Haven.
Danita Heard
Center Nurse ExecutiveWe are lucky to have Danita as our Center Nurse Executive. Not only does she have 20 years of nursing experience, but she has had the opportunity to work at several Genesis communities in Alabama and Georgia before coming to Elms Haven. She is currently serving on the Genesis Leadership Council and helping to mold strong future leaders within the company. A native to the south, Danita graduated cum laude from Jacksonville State University with a Bachelor’s of Science Degree in Nursing. Along with her duties at Elms Haven, Danita serves as the chairman of the community based organization Nurses Empowerment for Change. Their mission calls for nurses to be empowered to meet the challenges confronting the profession in an ever changing healthcare arena. This community based program provides education and free clinics to all.
Ashlee Cabral
Director of AdmissionsAshlee has been a vital member of the Elms Haven team for three years. She has valuable knowledge from her time in the business office and worked most recently as our Admissions Coordinator. Ashlee's favorite thing about working in health care is getting to know our residents and hearing their stories. Although she has lived in Colorado for over 20 years, she is originally a California girl from Lemoore. When she is not working on admitting residents from local hospitals and our community into Elms Haven, she is spending time with her two beautiful children, spoiling her many animals and catching up on her favorite TV series.
Christine Radomski
Director of RehabilitationChristine comes to Elms Haven with over 10 years of Physical Therapy experience. Chrissy received her Bachelor’s degree in Exercise Science and Physical Education from McDaniel College in Maryland and her license in Physical Therapy Assistance from Naugatuck Valley Community College in Connecticut. Chrissy moved out west four years ago to work at our sister facility, Powerback Lakewood, and Genesis’ home therapy program, Vitality to You. She transferred to Elms Haven two years ago and has found her home here. When she’s not caring for our residents, Chrissy spends time with her husband and two dogs. She also takes advantage of the Colorado sunshine by running, hiking and camping.
Here we are in the midst of a pandemic and I am so blessed that my grandmother is at Elms Haven. The staff and families have gotten together to make sure that residents are safe, well taken care of, given love, and attention. We have daily calls with updates on Covid-19. We have a family support group that helps each other during this time and provides snacks for the nursing staff. We love the staff so much and appreciate everyone at Elms Haven.10 stars for care, communication, and community!! Elms Haven is a nice facility. I adore all of the staff and appreciate them all so much. They take wonderful care of my grandmother and all of the residence there. Not only do they care for their needs but they really listen to their stories. I am there almost daily and really feel that they don't consider working there as a job but as a passion. Thank you all so much for your dedication!
