Welcome to Dunbar Center! We have been a part of the community of Dunbar, West Virginia for over 30 years. We are located in a peaceful country setting but still convenient to Interstate 64 and the Charleston area. Our center offers short-stay rehabilitation with separate specialized recovery suites as well as long-term care. Our therapy services include Physical, Occupational, and Speech Therapy. Dunbar offers full-time physician support, on-site Nurse Practitioner, and 24-hour nursing coverage. We offer comfortable rooms, numerous menu and dining choices, TV and internet access, and phone service in all rooms. The beauty shop is open twice a week for salon and barber services. Shawn Farley, M.H.A., L.N.H.A., C.T.R.
AdministratorShawn is originally from Plattsburgh, New York. He joined the military at the age of 17 and served in Germany as a member of the Air Force. He settled down with his family in West Virginia, where his father was born and raised. Shawn graduated from the University of Maryland in1998 with a minor in Business Management. In 2005, he obtained his Master's degree in Healthcare Administration. He has extensive professional background in Public Heath as the Director of Cancer Epidemiology followed by the director of the Office of Epidemiology and Prevention Services for the West Virginia Bureau for Public Health.
Shawn is a licensed Nursing Home Administrator and is currently over-seeing his third location within the Company. He leads at the center by being directly involved and getting to know the residents personally. He makes it a priority to conduct daily rounds and visit with each resident on a daily basis when at the center. He leads his team professionally and by example while working to ensure positive outcomes for all of the residents.
Out side of work, Shawn enjoys time with his family which includes his grandchildren.

Dr. James Toothman, MD
Medical DirectorDr. Toothman is a graduate from West Virginia University School of Medicine in Lewisburg WV and has been practicing in long- term care for 18 years. He previously worked in the Emergency Department at Pleasant Valley Hospital and St. Mary's Hospital in Huntington, WV. In keeping Dunbar Center’s residents and staff protected during the COVID-19 pandemic, Dr. Toothman reminds the staff of the importance of wearing full PPE at all times, screening employees at the point of entry, hand washing and social distancing among many other things that help keep our residents safe.

Christi Roach, APRN, FNP-BC
Nurse PractitionerChristi has worked at Dunbar Center for almost 3 years and works at center 5 days a week. She graduated from the University of Charleston in 1994 with a Bachelor’s degree in Nursing, Marshall University in 2010 with a Master’s Degree in Nursing and accredited through the ANCC for Nurse Practitioner. Christi has two daughters and one son whom she adores.

Tracy Pawnee Leggins, R.N
Center Nurse ExecutiveTracy has been a nurse for 31 years. She is originally from WV but moved to South Dakota in 2005. She recently moved back to WV to help on the family farm. She has spent many years working in the ER, ICU and has done flight nursing. She became a Director of Nursing in 2012 and has worked in acute care and long term care settings. Tracy has also received her RAC-CT certification and has held MDS positions. She has come to appreciate and admire the work that is carried out in the geriatric setting. She loves learning about each individual as she works with them, and who they are and what they did in life. “It is a blessing to care for the residents and their families”.

Lisa Snyder, LPN
Admissions DirectorLisa has worked in skilled nursing and long-term care for over 30 years. For 18 years she has spent her career working in Admissions and Marketing. Lisa has been at the Dunbar Center for over 20 years and is a member of the Genesis 20 year club. She remains active with the Alzheimer's Association and other organizations that act as advocates and resources for seniors. She loves balancing her professional career working in healthcare with that of spending time with her spouse Danney, children and many grandchildren. Lisa is dedicated to impacting the lives of patients and residents at the Dunbar Center.

William Moore, P.T.A.
Director of Rehab ServicesBill Moore has practiced physical therapy for the last 9 years throughout the tristate area treating our community in several settings. He graduated as a PTA from Mountwest in 2013. He is an enthusiastic and motivating member of the rehab department. Bills thrives on assuring that the therapy department works to accomplish patient goals and achieving the patients highest level of functional mobility. Bill has a son Carter and is engaged to be married to his beautiful fiancé, Sarah, who also works in healthcare. Bill enjoys sports, cars and working on home projects.
