Crestwood Center is an 81 bed facility located minutes away from the historical oval in Milford, NH. Crestwood can offer residential long-term care, respite care, hospice care, as well as therapy services for those patients who need rehabilitation before returning home.

Dr. Joseph De VeraMedical Director
Dr. De Vera is a Board Certified Internal medicine physician. He has seventeen years of experience as a Hospitalist that includes Catholic Medical Center and Monadnock Community Hospital. That experience provides Pheasant Wood Center residents a wealth of clinical expertise. Dr. De Vera also has eight years of outpatient primary care and wound care at both DHMC/Catholic Medical Center and Monadnock Internal medicine. As a member of the Genesis Physician Services he is also the Medical Director at the Pheasant Wood in Peterbourogh, NH. Dr. De Vera works very closely with our residents, their families and our staff.
Elizabeth Barnhill, RNDirector of Nursing
Elizabeth has been a Registered Nurse for 17 years. She has worked in Long Term Care for 16 of those years holding positions as Charge Nurse, Assistant Director of Nursing, and Director of Nursing. She has worked for the company since 1998 and has been with Crestwood Center for 5 years.
Anna LyonSenior Admissions Director
Anna graduated from Southern New Hampshire University in 2016 with a Bachelor of Science in Healthcare Management. She has enjoyed working in the healthcare field for several years now. Anna previously worked in a dual role at The Elms as the Director of Admissions and Social Services for 2 years. She was promoted to Senior Admissions Director for both Crestwood and The Elms in February 2021.
Amanda LeBlancDirector of Rehabilitation
Amanda went to school at Franklin Pierce University for both a Bachelors in Biology and then a Doctorate in Physical Therapy. She graduated in 2012 and has been working for Genesis ever since. She has also worked in home care but has preferred the SNF setting. She chose PT as a career as a sophomore in high school. She went to a technical high school in Massachusetts called Montachusett Regional Vocational Technical High School. It was there that she went through medical assisting training on shop weeks and the clinical instructor gave them an assignment to research the medical field for other areas they may be interested in. In her research she learned about Physical Therapy and this was really interesting to her. Initially, she only wanted to be a PTA. She was encouraged and given support by a clinical instructor from a co-op. She completed an outpatient PT rehab center to pursue her DPT. The rest is history! She enjoyed working with this population for both the challenges and the rewards she sees everyday.
