Crestwood Center is an 81 bed facility located minutes away from the historical oval in Milford, NH. Crestwood can offer residential long-term care, respite care, hospice care, as well as therapy services for those patients who need rehabilitation before returning home.

Hannah started her career with the Company during her Junior year of college when she interned for the summer. Hannah continued her path with us post-college and worked in HR and then pursued an opportunity to take an Administrator in Training position. Hannah is excited to continue her career with us and start her career as an Administrator with Crestwood Center. Hannah will continue to encourage new ideas, build positive relationships with residents, staff, and family, and be an advocate for all.
Dr. Ayobami OjutalayoMedical Director
Dr. Ojutalayo is a Board Certified Physician in Family Medicine. Dr. Ojutalayo graduated with an honors degree in biology from the Montclair State University in New Jersey after which he proceeded to acquire a Doctorate from the Ross University School of Medicine in 2007. He completed his Family Medicine Residency at the prestigious Dartmouth Medical Residency Program in New Hampshire. During his residency training experience, he developed a passion for teaching other residents and was awarded the best resident teacher in his program. Dr. Ojutalayo has been with the Company for the past five years.
Michelle Humphrey, NPNurse Practitioner
Michelle started her career in healthcare as an LNA in a Long Term Care Facility roughly 20 years ago. She found her true passion for caring for the elderly and continued her education and eventually became a Registered Nurse and working as a Director of Nursing. She has been the director of LTC centers, opened an Assistant Living/Memory care facility and directed a Home Health department for a CCRC. Michelle received her LPN at Mount Wachusetts Community College, RN/Associates from River Valley Community College, BSN from UMASS Amherst, and MSN/FNP from Mass. College of Pharmacology and Health Science. She starts here at Crestwood as a new Certified Family Nurse Practitioner with decades of nursing experience. Born and raised in Southern NH, Michelle resides in the area with her two children and dog. She enjoys music, art, and reading.
Amanda PerkinsDirector of Marketing and Admissions
Amanda has been with the Company since 2019 starting as a per diem LNA. In 2020 Amanda obtained a management position as the Staffing Coordinator/Payroll. In 2022 she became the Director of Marketing and Admissions. Amanda graduated in 2018 with a Master’s in Health Administration. In her spare time, Amanda can be found coaching her daughters for the local youth cheerleading association where she also serves on the board. She loves to travel and read. Amanda aspires to become an Administrator one day.
Amanda PeabodyDirector of Rehabilitation
Amanda went to school at Franklin Pierce University for both a Bachelors in Biology and then a Doctorate in Physical Therapy. She graduated in 2012 and has been working for Genesis ever since. She has also worked in home care but has preferred the SNF setting. She chose PT as a career as a sophomore in high school. She went to a technical high school in Massachusetts called Montachusett Regional Vocational Technical High School. It was there that she went through medical assisting training on shop weeks and the clinical instructor gave them an assignment to research the medical field for other areas they may be interested in. In her research she learned about Physical Therapy and this was really interesting to her. Initially, she only wanted to be a PTA. She was encouraged and given support by a clinical instructor from a co-op. She completed an outpatient PT rehab center to pursue her DPT. The rest is history! She enjoyed working with this population for both the challenges and the rewards she sees everyday.
Patricia PriveDirector of Nursing
We are excited to welcome Patricia as our new Director of Nursing. Patricia joins us at Crestwood Center transitioning from another affiliated facility, Harris Hill. Her 35-year nursing career has been based on the care of the elderly population and ranged from direct care to infection prevention and Assistant and Director of Nursing. Her goal is to take “old school” knowledge and blend it with new technology to support the nursing staff at Crestwood Center. Advocating for our residents and staff is always a priority.
Emily MorehouseDirector of Social Services
We are excited to welcome Emily Morehouse as our new Director of Social Services. Emily joined us in 2023 starting as Admission/Social Service Director at The Elms before transitioning to Social Service Director at Crestwood. She has been a Social Worker for eight years and was an LNA before starting as a Social Worker. On her days off she enjoys playing with her two sons and spending time outside.
