Country Village Center located in the White Mountains of New Hampshire, offers exceptional Skilled Nursing Care, LongTerm Care and Rehabilitation services. We offer semi-private and private rooms. We provide daily therapy in our 2,500 square foot therapy suite which offers state-of-the-art therapy equipment. We offer 24-hour food service with a choice of restaurant-style dining in our spacious dining room or an option for in-room dining. Our professional and compassionate staff develop daily routines to meet the needs and preferences of our residents. We offer a wide variety of activities and programs on a daily basis. Our caring staff goes above and beyond to make our residents comfortable. We offer a welcoming, home-like atmosphere and our staff treats all patients, residents and families like their own. Our goal is to ensure a safe return home for our ShortStay stay patients.

Erin Oleson
AdministratorErin is a dedicated mom of two wonderful kids and a passionate outdoor enthusiast based in Twin Mountain, New Hampshire. With a Bachelor's degree and over 19 years of experience in long-term care, she recently embarked on a new journey as a Nursing Home Administrator. Her career is complemented by her husband's role as a Captain in a local fire department, where they share a commitment to serving their community.
Outside of work, Erin’s family thrives on adventure. They love hitting the slopes to ski in winter, exploring trails for hiking, and embarking on exhilarating motorcycle trips during warmer months. Camping trips are a staple of their summers, where they embrace the beauty of nature.
Originally from Littleton, she has proudly called Twin Mountain home for the past 17 years. Engaged in community life, Erin actively contributes to the Fire Auxiliary and the Town Recreation Department, striving to make a positive impact on her neighborhood.
She cherishes every moment with her family, both at home and in the great outdoors, creating memories that fuel their love for adventure and community spirit.
Amber Schmidt, DO
Medical DirectorAmber received her medical degree from the University Of New England College Of Osteopathic Medicine and completed her Residency Program at Maine Dartmouth Family Medicine. She specializes in Primary Care, Family Medicine and Wound Healing.
Jenna Dutkewych
Occupational Therapist/ Rehabilitation Program ManagerIn 1996, Jenna graduated from the University of New Hampshire with a Bachelor of Science Degree in Occupational Therapy. Jenna has worked in a number of settings including skilled nursing, long-term care, acute care and pediatrics. She has been working with the organization since 2003 and in 2006, she became the Program Manager.
Jamie Frizzell
Social Services DirectorJamie graduated from Southern New Hampshire University in 2017 with a Bachelor’s degree in Human Services. Jamie has previously worked with adults with traumatic brain injuries, adults with drug and alcohol addiction, and joined our team at Country Village Center in 2022. Jamie’s continued goal is to provide resources to all residents, short and long-term, so their life is as fulfilling as they want it to be.
Nicole McKay
Admissions and Marketing DirectorNicole has been working in Long Term Care since 2014 and is a certified dementia practitioner. She attended the University of New Hampshire where she received her Bachelor’s in Psychology. She started her career as a caregiver and has worked in many departments within the same company, including scheduling, billing, and human resources. Nicole joined the team in June of 2023 as the Admissions and Marketing Director here at Country Village Center. We are so lucky to have her on our team!
Melinda Cotnoir, RN
Director of NursingMelinda was born and raised in the north country of New Hampshire in Berlin. She has worked in healthcare since high school. A previous operating room nurse for 10 years traveling the country and working at Dartmouth Hitchcock, Melinda decided to return to her roots in long-term care and give back to her North Country community. Melinda lives on her family's farm, which they have owned for over 100 years, and has three horses, which she rides and trains in her spare time.

"I want to take this opportunity to extend my profound and heartfelt thanks to you and your staff for the compassionate and very professional care you gave my 93 year old Dad, during his recent five week stay with you for rehab after his stroke.
I have long heard second hand of the legendary care you have given to various elderly friends in my church and community, but this was the first time I was able to witness it up close. From the moment my Dad arrived, your staff made him feel welcome and most efficiently took charge of his elaborate medications and many other personal requirements. I have great appreciation for this given the time and efforts both my wife, and our own care givers and I have had to give Dad here at home, who as you know is more or less completely immobile due to his long term Parkinson's disease. Its hard for me to imagine how you can apply this attention to some 86 residents simultaneously! However, that's exactly what you do!
As for the rehab program which was in particular directed to improving his speech and ability to eat on his own, that was beyond amazing. We all think he actually is now better off than he was before his stroke. This was further confirmed to me by the comments everyone outside the family made who stopped by to see him. I grew up visiting nursing homes from the very beginning of my life when I sometimes followed my Dad around on his late afternoon rounds to check up on his patients in various institutions in the Washington DC are. Even then and of course now as an architect working for various organizations including Northern Human Services, I have had a life long interest in envisioning the perfect environment for nursing facilities. Both my Dad and I agree yours is the closest to that ideal we have ever encountered. All of us and especially Dad himself extend our deepest thanks to your entire staff for all you did for him." "Just a few words to say Thank You to everyone on Notchway Rehab wing. Thank you for all that you did for me while in your care for four months. Each crew member played an important part in my recovery. Giving me courage to go on and making my life whole and at home again. The day shift and 3-11 shift I saw most of the time. The nurses were great and made me feel like a part of a family. This experience will be in my heart forever. The Rehab team worked hard to give me courage and compassion that I could do what I needed to when I went into the nursing home and I am now doing fine at home. The friendships and people I met I will be in touch with forever. The residents I will always remember God has followed me from beginning to end in the journey for my wellness. I just love it here...
My first "thanks" goes to my special "therapy" gals who were most helpful in getting me home and able to be active again. I wouldn't be going home if not for these gals. The love and friendships I have received from my wonderful friends here is so special and I really want to plan to see you all in the future and win at "BINGO" again and Fargo. I just love it here and now I have to go home, I cant take all of you with me with regret, but I will always give thanks that I was brought here to Country Village. It is a resort in disguise. The only regret I have is, I can't take all of you home with me.
