Carolyn Kear
AdministratorCarolyn began her career at Clayton Nursing & Rehab Center as a Business Office Manager in November 1998, and earned her Nursing Home Administrator license in September 2000, serving as Administrator for the last 18 years. During this time, our team has achieved 5 Deficiency Free surveys by the Department of Health. Carolyn's philosophy of healthcare: We strive to provide the absolute best possible quality care to our residents; we are responsive to the needs, concerns and preferences of our family members; and we care for and develop our employees.
Lori Dempsey, RN
Director of NursingLori has been with Clayton Nursing and Rehab Center since 1997, first as a charge nurse, than as MDS coordinator in 1999. In 2001, Lori has been in the position of MDS Coordinator and Director of Nursing Services. Since becoming an RN in 1996, she has worked in cardiology, and in 1997 starting working in long term care. Since working in this LTC facility Lori has had: 5 state deficiency free survey’s, 5 stars in Quality measures, high percentage of family and resident satisfaction on surveys. Lori enjoys Clayton's family environment and providing a high level of quality of care to our residents.
Melissa DeHerrera
Admissions DirectorMelissa has worked in healthcare for approximately 7 years. She has worked as a Certified Nursing Assistant in the skilled nursing environment. She also worked in Social Services and in the Activities departments as Directors of each. Currently, Melissa enjoys working as the Admissions Director at Clayton Nursing and Rehab because of the family-like atmosphere. She is motivated by making all residents feel at home.
Cassie House
Director of RehabilitationCassie House is an occupational therapist who has been practicing for over 18 years. Cassie graduated from the University of Oklahoma Health Sciences Center and has practiced in Oklahoma, Kansas, and now New Mexico, in many different settings. Cassie has experience working in outpatient therapy, inpatient rehab, school settings, home health, long-term care, and early intervention. Cassie started with Clayton Nursing & Rehab in August of 2021 as staff OT and serves as Director of Rehab. Cassie says that what she enjoys most about being an occupational therapist is building rapport with her patients and getting to know their stories and goals.
They care and love their residents
Five years ago, my sister, brother, and I faced the tough decision to put our dad in the nursing home. I called Clayton Nursing and Rehab, visited with Carolyn and asked if I could come by one day and visit with them. After visiting with them and touring the facility, I called my siblings and told them that this would be a great place for Daddy. Leaving him for the first time was very hard on him and us, but we also felt at peace knowing that he was going to be well taken care of. From that day until he passed away, he was given amazing care and was loved unconditionally. We cannot say enough about this facility. From the doctors, nurses, and aides to the office staff, janitorial, cafeteria, housekeeping, and maintenance workers, they all care about and love their residents. In Daddy’s last weeks and days, many of them would go in and sit and visit with him when we were unable to be there. Our friends at Clayton Nursing and Rehab will always hold a special place in our hearts. We will forever be grateful to each of them for the love shown and the care provided for our Dad.

Job well done
We had the pleasure of placing our mother, Phyllis Anson, in your facility in 2015. We tried to get her in Dalhart, but they didn’t have an opening. God was so good and brought the Clayton facility to our attention. She spent a year and a half there before going home to meet the Lord!
We could not have had better care. She was happy there. She met new friends. She loved the staff and they certainly loved her. I never witnessed an unpleasant odor in the facility, as I visited often as did my sister. Everyone was always treated with respect and kindness. We were encouraged to spend time there. My sister and I became family to the staff and many residents. I just want to thank you for taking awesome care of mother. I never worried about how she was treated or if she were fed or bathed. Thank you for making holidays special for her and for loving her as you all did.
I never questioned the decision we made to make Clayton Nursing and Rehab her home. Job well done, and Thank you.

Dear Carolyn, thank you so much for the summary. I was unable to be on Zoom this morning. You and your team desire much gratitude and recognition for your faithful commitment to serve the old, the weak, and the handicapped. It is challenging with COVID 19, but you persevere. And my brother, David, is blessed. Today he turns 76 which is a miracle after all he has been through since he was 15 with the horse accident and months of deep coma. Thanks again, and God bless you and the team.
