Etay Sahar, MSHCA, LNHA
Center Executive DirectorEtay started in Healthcare in 2010. His experience spans several levels of care including Skilled Nursing, Acute Rehab, and Assisted Living. He has a Master’s in Healthcare Administration from University of New Haven, CT. Etay has a strong passion for customer service and hospitality in health care. He has extensive experiences in labor relations and employee engagement. In his spare time, Etay enjoys spending time with his family, fishing, and riding his motorcycle.

Dr. Ana P. Gomes, DO, CMD, FAAFP
Medical DirectorDr. Gomes is a Graduate of the University of New England College of Osteopathic Medicine and was Chief Resident Family Practice at Warren Hospital in 1997. She went on to become a Certified Medical Director and Board Certified in Family Practice. Dr. Gomes has been practicing and serving Warren County for over 25 years with training in gynecological care and geriatrics. She has been working with diverse communities and loves spending time with her family. She enjoys travelling, farming, volunteering and bringing light to human challenges.
Donna Carducci, RN
Center Nurse ExecutiveDonna obtained her Diploma in Nursing in 1986 from The Allentown Hospital School of Nursing and has been a registered nurse for over 30 years. Aside from geriatric nursing, her clinical background includes acute cardiac nursing and ICU experience. She has been with Genesis HealthCare since 1989 in various roles including Infection Control, Education, and Nursing Administration. She is committed to provide quality care and achieve excellent outcomes.
Nicole Diomedo Barto
Director of Admissions and MarketingNicole has worked for Genesis HealthCare since 2014 as the Director of Admissions and Marketing, while Jessica started that role in 2016. Nicole obtained her Bachelor’s Degree in Psychology and has fifteen years of experience in healthcare. She has worked in a variety of settings including assisted living, mental health facilities and skilled nursing.

Erica Conroy
Director of RehabilitationErica obtained her undergraduate degree in Speech Language Pathology and Audiology from Stockton University. She then went on to earn her Masters in Speech Language Pathology from Misericordia University. She remains a recognized therapist by the state of NJ and is a national member of the American Speech-Language-Hearing Association (ASHA). Erica has worked for Genesis HealthCare Rehab Services since 2010 as a therapy team member and has been Director of Rehab since 2014.
Discharged home!
Bernice S. was admitted to Brakeley Park Center on 6/19/2019, following a right total knee arthroplasty status post distal femoral fracture while gardening with her husband. Upon evaluation, Bernice was ordered to partial weight bearing, which meant returning to home would take some rehabilitation. Bernice worked hard alongside her occupational and physical therapists in order to complete ADLs, stair training and functional mobility. Bernice agreed to complete stair training using compensatory strategies implemented by the team. This meant she would utilize her shower chair to safely negotiate 8 stairs (mimicking her home set up) while maintaining her weight bearing status. Bernice’s family attended caregiver training to learn the new technique prior to discharge. After a few weeks of significant progress and feeling confident, Bernice was discharged home; eager to set up her next trip to Florida!
Positive Outcome:
Andrew “Andy” P. is a resident of Brakeley Park Center and went through hell and back during a severe bout of COVID-19. He admitted to St. Luke’s Warren Hospital in early spring and spent over two months fighting for his life while being intubated for a total of 54 days. He suffered through high fevers, renal failure, and a slew of other comorbidities brought on by the virus.
Finally discharged and back home to Brakeley Park Center, Andy knew he had a long road of extensive rehabilitation to endure. Andy required the use of a total lift machine to safely transfer him and was totally dependent upon staff to feed and bathe him. “When I got home, I couldn’t even feed myself and I was on a pureed diet,” said Andy. He worked extensively with Erica Conroy to strengthen his swallow function so he could be upgraded back to a regular diet. The first solid meal he was able to consume successfully was a grilled cheese, which Andy stated “was like eating gourmet shrimp; it was so good having solid regular foods.” Eventually, he mentioned all he wanted was a T-bone steak, but he was not quite there yet. The entire team at Brakeley Park Center told Andy we would make that wish happen once he got stronger.
He worked tirelessly with occupational therapists Liz Wojcik, Natasha Carita and Darlyn Bawasanta of Genesis Rehabilitation Services (GRS) to regain strength in his dexterity to cut his own food, open containers and self-feed. Concurrently, he worked with physical therapists Eileen Militano, Tuesday Dy and Eun Rizzo of GRS to transfer and walk independently. Andy expressed that it was his in-room exercises with PT that provided him the motivation to keep going.
Once feeling better, it was time to get Andy that steak. Our first thought was to contact Rath’s Butcher & Delicatessen, owned and operated by Matt Johnson, in Stewartsville, NJ. Rath’s is a local establishment known for their superb product and dedication to the community. Matt Johnson graciously donated a massive tomahawk steak for Andy.
During the meal, Andy made a point to say “I’m here because I had a lot of good people behind me here and at the hospital. Thank you so much for this; you don’t know how much I appreciate it.”
