Richard Callister
Center Executive DirectorRichard (Rickey) Callister is excited to be a part of the Genesis team at Bloomfield. Prior to working in skilled nursing, he bounced between public health and hospital administration before finding a place in skilled nursing. Rickey enjoys being a part of good healthcare teams and helping patients and their families receive the best care. He enjoys spending time with his wife and two children.
Karina Holbert
Center Nurse ExecutiveKarina joined the Genesis team in 2018 at San Juan Center in Farmington and recently transferred to Bloomfield Genesis in October 2020 as CNE, she was previously a Unit Manager at San Juan Center, where she was mentored by their excellent administrative team who prepared her for this transition. She grew up in Farmington and is a graduate of Farmington High School. She attended nursing school in Salt Lake City and then worked at a respiratory hospital for 9 years before relocating back to the Four Corners area. She feels it is a privilege to care for our Elders and treats our residents as she would her own family and expects the same from her staff. She is the mother of 5 great kids and has family in the area. She is excited to be at Genesis Bloomfield working with a welcoming and compassionate team.
Victoria Alonzo
Director of RehabilitationVictoria has been at Bloomfield nursing and Rehabilitation center since 2014 first as a PTA and later as DOR. Specializing in short term, long term, and transitional care. Pursuing a career in LTC is what she has always caught her interest. Patient care is always number one to her and her team here in Bloomfield with a family like atmosphere within our residents and colleagues.
Tyson Collins
Director of Marketing & AdmissionsTyson Collins “Ty” is our Marketing and Admissions Director, he has worked with Bloomfield Nursing and Rehabilitation for over 6 years. He started his career in Long Term Nursing in 2007 as a Certified Nursing Assistant, he was drawn to the geriatric patients because he believes in honoring our elders and caring for them in their final stages. Ty then went on to become the Activity Director in 2015, he enjoyed working with the residents and hearing stories of their life journeys. He also tried his best to incorporate traditional Native American and Hispanic activities into our center’s diverse culture. Ty became our Marketing and Admission Director in 2019, he has enjoyed getting to know our community members and letting them know about our many changes and future endeavors for our center. Ty has always been a people person and enjoys meeting new people, he looks forward to helping our center achieve more success for years to come. Ty was raised in Farmington, NM and graduated at FHS, he resides in Bloomfield NM. He is proud of his Laguna Pueblo (Sun Clan) and Navajo (Many Hogan Clan) background. He is a parent to six fur babies, three dogs and three cats. When he is not working at the center he spends time with his family throughout New Mexico. He has always been a social butterfly and is passionate about caring for others. He looks forward to meeting family and friends of our residents. If you're in the area stop by for a tour he will gladly show you what our center has to offer.
Good experience and polite staff
I really had some good learning experiences about some ways of exercise and physical therapy. All nurses were very nice and polite. Food was pretty good. This facility is a lot better than others. I'd just like to say thanks to all staff and keep up the good work!
Proud of our Therapy Department!
We are proud of our Therapy Department with all their countless hours of dedication and hard work to help improve our residents' lives here at Bloomfield Nursing & Rehabilitation. We offer Physical Therapy, Occupational Therapy as well as Speech Therapy.
One of our residents who wished to remain anonymous would like to share their success story with our therapists. Our long-term care resident was in a motorcycle accident when he suffered a cervical neck injury which resulted in quadriplegia. This had left him requiring a geriatric chair due to the severity of his functional limitations. He first entered physical therapy to improve strength, range of motion, and build endurance, and occupational therapy adjusting to help with adjusting to everyday life. Occupational Therapy focuses on activities of daily living, eating, toileting, and grooming. However, his recovery outcome required that he receive total assistance from a staff member 24 hours a day for his personal care.
After being discharged he received restorative nursing care to maintain his strength and mobility. The physical and occupational therapy team re-intervened with him as he later was seen showing some positive results in muscle function, balance, and sitting tolerance as some of his functions had shown some recovery. His goal in therapy was to be able to one day gain some independence and sit in a wheelchair so he could participate in his activities of daily living. The therapeutic rehabilitation team developed a specific therapeutic intervention regimen to meet his physical strength, mobility, and his emotional and individual needs.
In several short months, with intensive training he slowly transitioned from a geriatric wheelchair to a high back recline wheelchair with supportive wheelchair accessories for safe positioning and adaptive equipment to assist with his stand transfers to and from his wheelchair with our trained nursing care staff. He is now able to perform a stand turn transfer with a two-person transfer to and from his wheelchair. Demonstrate good posture during meal times having improved access to independently drink from a cup, and improved mobility for occupations of choice improving his overall self-esteem and independence. He stated that, “he is happy with his new wheelchair.” He continues to receive restorative nursing care to maintain his current level of function and is seen within the facility feeling good about the personal goals that he had set for himself.

I had 2 great nurses, LeGay and Tess who were outstanding! Jackie was helpful in getting rehab and discharge. Sheri and Tom in therapy were also great. I received first class care!
