Vivian Del Toro
Center Executive Director

Willow Creek Healthcare Center Administrator Vivian Del Toro has a master’s degree in education and earned her Administrator's license in 2008. Soon after, she became the administrator of Willow Creek Healthcare Center. In total, Mrs. Del Toro has 18 years in the skilled nursing industry and holds very high standards for the facility. She prides herself as an administrator who really cares for the needs of the patients and residents, as well as creating a positive and family oriented atmosphere which has resulted in longevity of her staff.

Christina Flores
Admissions Director

Willow creek Healthcare Center Admissions Director Christina Flores has been in the skilled nursing for 15 years. Her career began as a certified nursing assistant, CNA, and then moved on as a Physical Therapist/Occupational Therapist aide. Due to her strong work ethic and responsible nature, Christina was promoted to become the Admissions Director and has held that role for the last 6 years. Christina loves helping people and says that working in admissions can be extremely demanding, but at the same time very rewarding when you make a difference in someone’s healing process.

Angela Amparado
Director of Nursing

Willow Creek Healthcare Center Director of Nursing Angela Amparado has had a long and rewarding nursing career and has served as Director of Nurses for the last 6 years. After earning her bachelor’s degree in Nursing, Angela began working as a certified nursing assistant then became a registered nurse. Due to her diligence as a nurse, Angela then became Charge Nurse, Nurse Supervisor, Assistant-DON and presently Director of Nurses. Angela is very dedicated and loyal to the organization.

Elizabeth (Liz) Chuhlantseff
Director of Rehabilitation

Liz is a licensed Medical Speech-Language Pathologist who graduated from University of Nevada, Reno Medical School in 1992.  She gained advanced medical training in orthopedics and cardio-pulmonary rehabilitation services with working experiences coming from Level One Trauma, Acute Rehabilitation, Outpatient Services, Private Practice and Home Health Services across pediatrics to geriatrics.  She specializes in dysphagia, aphasiology, traumatic brain injuries, neurologic deficits, voice disorders, cognitive related disorders (dementia).  Her knowledge and valued based services in the health care industry, where change is inevitable, dominants the philosophy of function.  Her enthusiasm and compassion can be summed up by hearing her say, “I believe we all have the ability to function at a level to have our wants and need met when given the opportunity to remediate, restore and adapt to any circumstance.”
