Yong Crandall, RN, BSN, LHA
Center Executive Director

Yong recently returned to the Village Green of Bristol as the Center’s Executive Director.  She brings a wealth of knowledge and experience from her Nursing career.  Yong started here years ago as a floor Nurse.  She has worked her way through various positions, to become the Director of Nursing.  Her return to the team has brought a revived sense of vitality.    Yong has an “open door” policy and is a very familiar face to residents, families and staff throughout the building.  Yong promotes the day-to-day teamwork and compassion for all at The Village Green.

Pamela Leone, RN, MSN, CDE
Center Nurse Executive

Pam joined the Village Green team in June, 2016.  Pam has had a seven-year career as a nurse, working her way up from charge nurse to Unit Supervisor, Unit Manager, Nurse Practice Educator and Assistant DON, before becoming a Center Nurse Executive.  Pam’s down-to-earth and hands-on approach to her position is refreshing and supportive for both staff and families.  Pam’s goal is to provide the highest quality clinical care possible for all patients at Village Green.

Kathleen Payne
Director of Admissions

Kathy has been a member of the Genesis family for seven years.  She has served as an Admissions Director in several Genesis buildings throughout Connecticut and Vermont.  Kathy works with other team members to create a welcoming and vital environment for patients and residents.  Kathy has forged strong relationships with the local hospitals, Chambers of Commerce and health care providers always striving to keep them informed about clinical initiatives in the building and the high-quality care provided at the Village Green of Bristol.

Thomas Jackson, MSPT
Director of Rehabilitation

Tom has been a physical therapist for ten years, serving as a Director of Rehabilitation for the past seven years.  Tom joined the Village Green team in 2015.  He has worked in acute care and outpatient settings, but his passion is for the fields of geriatrics and skilled nursing.  Patients are Tom’s number one priority.  He prides himself on instituting a rehabilitation program to help patients reach their goals and provide them with the best possible therapeutic outcomes.
