We offer a specialized Transitional Care Unit for patients needing Rehabilitative Care or continued Medical Management after a hospital stay. Our professional, dedicated and compassionate staff want to assist you in the development of your individualized treatment program. Sarah S. Brayton's on-site nurse practitioners and attending physicians work together with our staff to be sure you receive the best medical care possible as we work to get you back to your home quickly, and at the most independent and safest level as possible.
